Study On The Curative Effect Of Two Modified Methods In The Treatment Of Lower Extremity Varicose Veins With Laurinol Sclerotherapy

Objective:To compare the efficacy of two modified ultrasound-guided foam sclerotherapy of lower limb varicose veins,and to find a more effective method of minimally invasive sclerotherapy.Methods:In the first part,16 patients with 22 cases of lower limb varicose veins CEAP grade c2-c5 admitted to The First Affiliated Hospital Of Kunming Medical University and Zhaotong Hospital of Traditional Chinese Medicine were collected by retrospective study and analysis method.Respectively under the UGFS and the MUGFS.One week,three months and half a year after the operation,clinical observation(general condition,appearance of the affected limb,complications,VCSS)and ultrasound examination(two-dimensional,color doppler)were performed.To evaluate the number of puncture needles,therapeutic effect and postoperative complications of these two treatments for lower limb varicose veins.In the second part,26 patients with 28 cases of lower limb varicose veins with grade c2-c5 admitted to The First Affiliated Hospital Of Kunming Medical University and Zhaotong Hospital Of Traditional Chinese Medicine were collected through retrospective study and analysis.Among them,11 cases with smaller varicose internal diameter received MUGFS.UC+MUGFS was applied to 17 patients With large varicose internal diameter.One week,three months and half a year after the operation,clinical observation(general condition,appearance of the affected limb,complications,VCSS)and ultrasound examination(two-dimensional,color doppler)were performed.To evaluate the efficacy and postoperative complications of these two treatments for lower limb varicose veins.Results:In the first part,the treatment success rate of the two groups was 100%,and there was no obvious bleeding during the operation.The average needle count of UGFS group and MUGFS group was 12.5±4.9 and 4.9±1.1 respectively(P<0.01,the difference of needle count between the two groups was significant).In UGFS group and MUGFS group,there were 7 cases(63.6%)and 8 cases(72.7%)varicose veins disappeared in the affected side,4 cases(36.4%)and 3 cases(27.3%)varicose veins in the affected side decreased significantly.Complications:no serious complications were found in this study.The incidence of complications in UGFS group and MUGFS group:Hyperpigmentation:63.6%and 45.5%;Local pain:27.3%,18.2%;Subcutaneous congestion:18.2%,18.2%(P>0.05,the difference was not statistically significant).Efficacy:the recurrence rates of UGFS group and MUGFS group were 27.3%and 9.1%,6 months after surgery(P>0.05,with no statistical significance).Six months after surgery,both groups of patients were satisfied with the cosmetic effect.In the second part,the treatment success rate of the two groups was 100%,and there was no obvious bleeding during the operation.In MUGFS group and UC+MUGFS group,there were 8 cases(72.7%)and 9 cases(52.9%)with disappeared varicose veins in the lower limbs on the affected side,3 cases(27.3%)and 8 cases(47.1%)with significantly reduced varicose veins in the affected side.Complications:no serious complications were found in this study.The incidence of complications in MUGFS group and UC+MUGFS group:drunk reaction:0%,5.9%;Hyperpigmentation:45.5%,47.1%;Local pain:18.2%,11.8%;Subcutaneous congestion:18.2%,5.9%(P>0.05,the difference was not statistically significant).In terms of effectiveness,the recurrence rates of MUGFS group and UC+MUGFS group were 9.1%and 0%,6 months after surgery(P>0.05,with no statistical significance).Six months after surgery,both groups of patients were satisfied with the cosmetic effect.Conclusions:MUGFS significantly reduces the number of needles and the patient suffers less than UGFS.In addition,the therapeutic effect of varicose veins in lower limbs is similar to UGFS.Patients with MUGFS+UC for severe varicose symptoms had the same effect as those with MUGFS for mild varicose symptoms.The three methods for the treatment of lower limbs varicose veins are characterized by low recurrence rate,less complications and high self-assessment satisfaction of patients,and are safe,minimally invasive and effective.
