PurposeTo investigate the adverse reactions of breast cancer patients after chemotherapy,and to explore the effects of music intervention on their immune function and quality of life by implementing music intervention to some patients with breast cancer chemotherapy,in order to provide effective methods for alleviating the side effects of breast cancer chemotherapy and improving the quality of life of the patients.MethodsThe clinical data of 102 patients who were hospitalized in the breast surgery of Affiliated Hospital of Tai'an Medical College from November 2016 to March 2018 were diagnosed as malignant breast tumors by histopathological examination,after modified radical mastectomy and periodic chemotherapy.All patients were female and divided into 2groups by random sampling.52 cases in the control group.Age(28-70)years,average age(49.1±5.3)years,28(53.85%)before menopause,24 cases(46.15%)after menopause,20 cases(38.46%)in high school and above high school,32 cases(61.54%)below junior high school and junior high school,occupation status for farmer 33(67.46%),fixation work and freelance Case(17.31%);Pathological features,tumor size:10 cases(19.23%),29 cases(55.77%),more than 5cm13(25%),lymph node metastasis:no lymph node metastasis 30 cases(57.69%),1-3 17 cases(32.69%),4-9 4 cases(7.69%),or more than 29 cases(32.69%);TNM stage:phase I cases,II.There were 32 cases(61.54%)and 11 cases(21.15%)in stage III.Immunohistochemical features:ER(-)/(+),negative 4 cases(7.69%),(++)/(+++)that was positive in 27 cases(51.92%),PR(-)/(+),negative 9 cases(17.31%),(++++),45cases(86.54%),14 Ki-67 positive cancer cells,14(26.92%),>20%,high expression 38(73.08%),and Her-2(-)/(+)negative 2 6 cases(52%),(+++)that was positive 7 cases(13.46%),Her-2(++)need to walk in situ fluorescence in situ hybridization(FISH)detection clearly have no gene amplification,Her-2 gene amplification is positive 5 cases(9.62%),Her-2 gene no amplification is negative 11 cases(21.15%);Molecular typing:Luminal B HER-2 negative 25 cases(48.08%),Luminal B HER-2 positive type 13 cases(25%),HER-2 positive 10 cases(19.23%),Basal-like type 4 cases(7.69%);Pathological classification:38 cases(73.08%)of invasive ductal carcinoma(infiltrating ductal carcinoma,IDC),10 cases(19.23%)of invasive lobular carcinoma(infiltrating lobular carcinoma,ILC),and 4 cases(7.69%).50 cases in the intervention group.Age(29-70)years,average(50.3±5.2)years,30cases(60%)before menopause,20(40%)menopause,24(48%)in high school and high school,26(52%)below junior high school and junior high school.The occupational status was 36 cases(72%),10 fixed work(20%)and 4 freelance(8%),pathological features,tumor size:less than 2cm 6 cases(12%),2-5cm 30 cases(60%),or more than 5cm14 cases(28%);lymph node metastasis:no lymph node metastasis 29 cases(58%),metastasis of1-3 cases(28%),4-9 5 cases(10%),more than 10 2 cases(4%),TNM stage:stage I 10cases(20%),20 cases(40%)in stage II,20 cases(40%);Immunohistochemical features:ER(-)/(+),negative 2 cases(4%),(++)/(+++)that is positive 21 cases(42%);PR(-)/(+),negative 8 cases(16%),(++++),that is,40 cases(80%);Ki-67 positive staining cancer cells accounted for 10 cases of low expression of<20%,40 cases of high expression of>20%(80%),Her-2(-)/(+)negative 30 cases(60%),(+++),10 cases(20%),Her-2(++)FISH detection of Her-2 gene amplification in 4 cases(8%),Her-2 gene no amplification abnormality that negative 9 cases(18%);Molecular typing:Luminal B HER-2 negative 31cases(62%),Luminal B HER-2 positive type 10 cases(20%),HER-2 positive 7 cases(14%),Basal-like type 2 cases(4%);pathological classification IDC 35 cases(70%),ILC10 cases(20%),other 5 cases(10%).Compared to baseline,there was no significant difference between the two groups of patients'immune function,quality of life(functional quality and chemotherapy response symptoms)(P>0.05),and clinical comparability.Chemotherapy regimens were combined with docetaxel,pirarubicin and cyclophosphamide,that is,TAC regimen,a cycle of 21days.Explain the purpose and operation method of music intervention to patients,and get informed consent from patients.The music is selected from the Chinese traditional five line music,which is published by the Chinese Medical Association electronic video publishing house,including the"ten face buried","three fold of Yang Guan","Eighteen shots of Hu","Zizhu Tunes","plum blossom three"and so on.Convert CD music into MP4 mode.Two groups of patients were treated with the same regimen of chemotherapy,and the intervention group was given the same type of player with the same type of music intervention within 0-1.5 hours at the same time,and 4 cycles were intervened.The operation was carried out in the patient's ward.The peripheral venous whole blood 3ml was extracted 1 days before chemotherapy and 4 cycles before chemotherapy.The specimens were sent to the Affiliated Hospital of Tai'an Medical College laboratory department to determine the distribution of T lymphocyte subsets and immunoglobulin content in peripheral blood of the patients,and to fill out the quality of life assessment scale(QLQ-C30)of the patients with breast cancer.The data were processed by SPSS23.0statistical software package.ResultsBefore chemotherapy,there was no significant difference between the two groups of patients,such as age,menstrual state,pathological features,immunohistochemical features,molecular typing and so on,and was comparable(P>0.05).There were 99 cases of all the results evaluated by this clinical trial,of which 50 cases in the intervention group,2 cases withdrawn from the study,3.92%of the local hospital chemotherapy,and 49 cases in the control group,1 cases were withdrawn from the study because of their own reasons,accounting for 1.96%.In group comparison,the score of CD4+and CD4+/CD8+and IgG,the score of body function and emotional function in the intervention group were higher than that before chemotherapy(P<0.05 or 0.01),and the scores of self-conscious symptoms of nausea,vomiting,insomnia and anorexia were decreased,the difference was statistically significant(P<0.05 or 0.01).After chemotherapy,the values of CD4+,CD4+/CD8+and IgG in the control group were lower than those before chemotherapy,and the scores of body function and emotional function were significantly reduced.The patients'self-conscious symptoms,nausea,vomiting,insomnia,and loss of appetite,were more serious than before chemotherapy,and the score increased,the difference was statistically significant(P<0.05 or 0.01).The contents of CD3~+,CD4~+and immunoglobulin IgA and IgG in the control group were all lower than those before chemotherapy,and the scores of function and quality were significantly decreased.The patients'self-conscious symptoms were significantly worse than those before chemotherapy,and the score increased(P<0.05 or 0.01).After chemotherapy,the KPS score in the intervention group was 29(59.18%),13(26.53%)and 7(14.29%);the KPS score in the control group was 34(48%),5(10%),and21(42%).The KPS score of the intervention group was lower than that of the control group,and the difference was statistically significant between the two groups(P<0.05).After chemotherapy in the two groups,the incidence of leukocyte decline was 46%in the intervention group and 26.5%in the control group.The difference was statistically significant(P=0.029<0.05).The incidence of impaired liver function was 42%in the intervention group and 28.6%in the control group,respectively(P=0.041<0.05).Conclusion1.Music intervention has a positive effect on the regulation of immune function of breast cancer patients with chemotherapy.2.Music intervention can obviously alleviate the adverse reaction of chemotherapy in breast cancer patients.3.Music intervention during breast cancer adjuvant chemotherapy can improve the quality of life of patients.
