Objective: To investigate the relationship between the HGI of T2 DM and diabetes-related complications and clinical application.Methods: The clinical data of 776 patients with T2 DM who were hospitalized in the endocrinology department of our hospital were collected.After calculating the HGI,the median HGI level-0.35 was used as the cutoff value.It was divided into 388 patients with low HGI(HGI<-0.35)and high HGI(HGI≥-0.35)group 388 cases,compared the differences in related indicators between the two groups,and analyzed the correlation between HGI and the T2 DM related complications.Results: There were no significant differences in gender ratio,age,TG,HDL-C,apoA,Lpa,GGT between the two groups(P>0.05).There was no significant difference between the two groups in terms of insulin dose,ratios of OADs,the prevalence of hypertension,CHD,DR,and,carotid arteriosclerosis(P>0.05).HbA1 C,FPG,HGI,TC,LDL-C,apoB,LDH,duration of diabetes,statin and insulin users,DPN and LEAD prevalence were higher than Low HGI group(P<0.05),total bilirubin level was lower than low HGI group(P<0.05).HbA1 C and FPG were positively correlated in T2 DM patients(P<0.01).HGI(OR=1.112,P=0.037),age(OR=1.050,P<0.001),duration of diabetes(OR= 10.27,P<0.001),combined with DR(OR=1.650,P=0.005)are risk factors for DPN in patients with T2 DM.The risk in the high HGI group was 1.696 times higher than low HGI group.HGI(OR=1.189,P<0.001),age(OR=1.069,P<0.001),combined carotid atherosclerosis(OR=1.747,P=0.001)are risk factors for LEAD in T2 DM patients.The risk in the high HGI group was 1.870 times higher than low HGI group(OR=1.870,P<0.001).Conclusion: Patients with high HGI have worse control of blood glucose,lipids,and inflammatory response.Increased HGI is associated with an increased risk of DPN and LEAD in T2 DM patients.
