| Objective:To evaluate the effect of His bundle pacing therapy for chronic heart failure.Methods:Search PubMed,EMbase,The Cochrane Library,CNKI,Wan Fang,VIP,SinoMed to obtain clinical trials of patients with heart failure treated with His bundle pacing,time is before August 2019.Two reviewers independently screened the literature,extracted the data and evaluated the risk of bias in the included studies and performed meta-analysis using Review manage 5.3 software.Results:A total of 15 studies were included in a total of 500 patients.The overall success rate of His bundle pacing implantation was 83.8%(419/500).Compared with baseline,there were significant differences in QRS duration was shortened[MD=37.36,95%CI(25.29~49.43),P<0.001,I~2=95%],left ventricular ejection fraction was increased[MD=-11.96,95%CI(-14.54~-9.38),P<0.001,I~2=70%],NYHA cardiac function grading was improved[MD=1.24,95%CI(0.98~1.50),P<0.001,I~2=89%],pacing threshold was increased[MD=-0.32,95%CI(-0.47~-0.18),P<0.001,I~2=11%],and pacing impedance was decreased[MD=82.12,95%CI(28.48~135.75),P<0.05,I~2=87%]at follow-up.Conclusion:His bundle pacing is an effective method for the treatment of chronic heart failure,which can reduce the patient’s QRS duration and significantly improve the patient’s heart function.However,due to the preliminary research stage of His bundle pacing,a large number of randomized controlled trials are needed to evaluate its long-term efficacy and prognosis.(This study has been registered in the systematic review of the internationalized prospective database PROSPERO,ID:145326)... |
