| Objective: To determine and count the incidence of osteoporosis(op)in changchun community and the distribution of traditional chinese medicine constitution,to investigate the correlation of bone mineral density,bone metabolism and blood biochemistry index and physique type of traditional chinese medicine,to clarify the influence factors of op and its correlation with physique type of traditional chinese medicine and the time point of early intervention.Methods: A multi-stage cluster stratified random sampling method was adopted to extract 1786 resident middle-aged and elderly residents from 45 communities in Changchun from October 2017 to June 2018,and to fill out the questionnaire of national osteoporosis syndrome combined with prospective community cohort study case report.According to the standard survey of TCM physique classification and judgment,physique type of traditional chinese medicine were counted.The lumbar spine and left hip density(BMD)were measured by double-ray X-ray absorption method with T and Z values,and the degradation products of beta-collagen(β-CTx),osteocalcin(OST),creatinine(CR),parathyroid hormone(PTH)and alkaline phosphatase(ALP),blood phosphorus(P),blood magnesium(Mg),blood calcium(Ca),Type I procollagen amino-terminal peptide(PINP),25-hydroxy vitamin D25(OH)D,to analyze the correlation between bone metabolism index and traditional chinese medicine constitution.To analyze the correlation between OP and Constitution type of traditional Chinese medicine and related risk factors.Results: A total of 1786 subjects were included,the average age was 65(64.719±8.149)years old,of which 454 were male,1332 were female,343 were normal,831 were decreased and 612 were loose.Studies have shown that the older you get,the heavier your bone loss.The loss of bone mass in women was more severe than in men,and the difference was statistically significant(P<0.05).Household income and the frequency of leftovers were negatively correlated with bone mass,and night sleep time was positively correlated with BMD.The smaller the CR was,the more BMD was lost,and the lower the β-CTx was.The larger the values of OST,CR,PTH,ALP,P,Mg,PINP,the more BMD lost,the positive correlation.The contents of total Ca and 25(OH)D were not different in all age groups,and the statistics were meaningless(P>0.05).after collinear diagnosis,it was found that β-ctx,PTH,ALP,P,Mg,Ca,25(OH)D had strong collinearity.after excluding the confounding factors,age,sex,OST,PINP were the influencing factors of bmd and showed a negative correlation;CR,weight were also the influencing factors of bmd,but positive correlation.Under the same circumstances,femininity is at greater risk of developing OP than men.The proportion of bone mass in normal population was 19.2%,and the number of the other 8 kinds of biased physique population was not statistically different(P>0.05).The bone mineral density of lumbar vertebrae and hip of each physique type fluctuated around the peak bone mass.The detection rate of osteoporosis was 34.3%,and five kinds of partial constitution were affected by OP,according to OR From big to small,they are phlegm wet quality > special quality >yin deficiency quality > qi deficiency quality > yang deficiency quality.Compared with Pinghe quality,the risk of qi deficiency quality is 3.5 times higher than that of osteoporosis,2.9 times of yang deficiency,4.4 times of yin deficiency,7.8 times of phlegm dampness and 5.7 times of special quality.Conclusion: The detection rate of primary osteoporosis among middle-aged and elderly residents in Changchun city is 34.3%,which is higher than the national average.In daily life,the middle-aged and elderly people in Changchun city should increase their outdoor exercise time and exercise quantity,reduce the habit of eating leftovers properly,reduce the frequency of eating pickled food,and keep enough sleep at night.The physique type of the normal population of bone mass in every community of Changchun is coordinate physical,and it is preliminarily considered that harmonize corporeity is the protective constitution of OP,and biased constitution is an important risk factor for osteoporosis.Most of the unfair physique types were mainly characterized by Yang deficiency,Qi deficiency and Yin deficiency.Elderly in the community In the primary osteoporosis screening prevention and treatment,it is recommended that OST,PINP,CR as the three bone metabolism indicators as routine detection.Using "TCM physique identification" to treat phlegm dampness,qi deficiency,yang deficiency,yin deficiency and special constitution as the key prevention and treatment objects of abnormal bone mass. |
