ObjectiveAs in recent years in technology and equipment of continuous improvement,percutaneous coronary interention(PCI)from the initial percutaneous coronary artery lumen keratoplasty(PTCA)into the stent era,however,stent eyewinker residue to endothelial healing delay,bring in-stent restenosis(ISR),stent thrombosis,and other issues.Drug-coated balloon(DCB)is PCI treatment of cardiovascular diseases(CVD)a new revolution,and with the optimization of DCB indication,DCB increasing application in the real world.Therefore,it is important to select high-risk patients by using indicators convenient for detection in the work,and to predict and intervene the occurrence of postoperative adverse events in advance.Recent studies have shown that fibfinogen(FIB)is closely related to the occurrence,development and prognosis of coronary artery diseases,while there is little research evidence on the relationship between FIB and the prognosis after coronary intervention,especially after DCB.The purpose of this study was to investigate the clinical value of plasma FIB in determining the prognosis of DCB in the treatment of coronary artery lesions.MethodsFrom June 2018 to June 2019,84 patients who only received DCB dilatation for coronary artery stenosis in the second affiliated hospital of zhengzhou university were collected.Records all the general clinical data and into the group of patients underwent percutaneous coronary interention baseline characteristics of the data,by telephone,outpatient follow-up observation,hospital be in hospital information query,according to the prognosis will be hospitalized for recurrence of angina pectoris,target lesion reascularization,experienced major adverse cardiac events(including cardiac death and myocardial infarction,heart failure,malignant arrhythmia),set the patients for all-cause mortality as poor prognosis group(n=23),set the the rest into the group of patients as control group,the group(n=61)good prognosis.SPSS20.0 statistical software was used for data analysis to compare the differences in plasma FIB concentration and Gensini score between the groups with poor prognosis and those with good prognosis.Results1.In the general clinical data and blood biochemical indicators and prognosis between good and poor prognosis group in age,diabetes history,history of myocardial infarction,HbA1C,Scr,FIB and Gensini score between comparison indicate statistically significant differences(p<0.05),while other factors,such as follow-up time,gender,history of high blood pressure,smoking history,the WBC and NEUT%,RBC,HGB,PLT,GLU,ALT,AST,TC,TG,HDL,LDL-C-C comparison indicate differences had no statistical significance(p>0.05);2.Spearman correlation analysis between plasma FIB and Gensini score showed that the two were positively correlated(r=0.359,p=0.001);3.Whether to DCB postoperative prognosis events as the dependent variable,each observation indexes binary Logistic regression analysis of single factor,the results suggest age,history of myocardial infarction,HbA1C,Scr,FIB and Gensini score p values<0.05 was statistically significant,further lines of multi-factor binary Logistic regression analysis of FIB and Gensini score is predicting coronary lesions DCB treatment the independent risk factors of poor prognosis.4.FIB was used as the test variable to draw the ROC curve.The results showed that the critical value of FIB in predicting the occurrence of adverse prognostic events after DCB was 3.025g/L,the sensitivity was 78.3%,the specificity was 62.3%,and the AUC under the curve was 0.732.Patients with FIB>of 3.025g/L were selected as the high-value FIB group and the rest as the low-value FIB group.Further comparison was made between the two groups on the incidence of adverse events after DCB surgery,and the difference was statistically significant(p<0.05).Conclusions1.Plasma FIB level in the poor prognosis group was significantly higher than that in the good prognosis group.2.Plasma FIB is positively correlated with coronary Gensini score.3.Plasma FIB and coronary Gensini scores are independent risk factors for predicting the occurrence of adverse prognosis in patients with coronary artery disease treated with DCB.
