Abstract/Summary:
Objective: To study the clinical effect of paclitaxel drug-coated balloon in coronary in de novo lesions.Methods: A retrospective analysis was performed between October 2018 and June2020 at the First Affiliated Hospital of Wannan Medical College of Wuhu and its cooperative medical consortium hospital.Coronary angiography was performed to confirm the diagnosis of coronary artery disease in de novo.A total of 120 patients were treated with a drug-coated balloon at the site of coronary artery stenosis.The quantitative measurement and analysis software of coronary angiography(QCA)was used to measure the coronary arteries of patients before,after and after the angiographic follow-up.Minimum lumen diameter(MLD).The patients were followed up clinically for 9-12 months.Follow-up content mainly includes the incidence of adverse cardiac events(MACE),target lesion revascularization rate(TLR),target lesion failure rate(TLF),mortality,etc.Results: The selected 120 target patients were 60.70±11.31 years old,of which 65(54.1%)were males,55(45.8%)were females,58(48.3%)had a history of smoking,and 47(39.1%)had a history of drinking Cases,76(63.3%)cases of hypertension,32(26.6%)cases of PCI,26(21.66%)cases of diabetes,48(40%)cases of stable angina,63(52.5%)cases of unstable angina There were 79(65.6%)patients with ECG ST-T changes or T-wave changes,23(19.2%)patients with abnormal Q wave,and 65(54.1%)patients with coronary angiography showing varying degrees of coronary vascular calcification.The average length of the patient’s coronary target lesion was13.26±6.05 mm,the average diameter of the pre-expanded balloon used was2.55±0.45 mm,the average length was 15.03±5.56 mm,and the average stenosis of the target lesion was 85.56±6.28.The average diameter of the drug balloon used was2.58±0.65 mm,the length was 20.06±5.65 mm,the average intraoperative filling pressure of the drug balloon was 8.56±1.5atm,and the average filling time of the drug balloon was 58.50±4.8s;immediate target lesion failure(TLF)Is 0(0.0%)cases.In the 9-12 months of clinical follow-up,the target lesion revascularization(TLR)is 3(2.5%)cases,and the non-target vascular revascularization is 5(4.16%)cases.Chest pain recurrence was 4(3.33%)cases,target lesion thrombosis was 0(0.0%),target lesion myocardial infarction was 0(0.0%),cardiogenic death was 0(0.0%),and MACE events were 7(5.8%)cases.The preoperative and immediate postoperative MLD measured by QCA were 0.56±0.45 mm and 2.53±0.56 mm,respectively.The difference between the two was significant and statistically significant(P < 0.01);the MLD of the patients following the angiographic follow-up was 2.28±0.50 mm.Comparing the MLD immediately after operation,there was no significant difference between the two,which was not statistically significant(P=0.76).Conclusion: Paclitaxel drug-coated balloon has certain safety and effectiveness in the application of coronary artery in de novo lesions.Given that DCB therapy has its own advantages in avoiding permanent stent placement in the body and reducing the need for antiplatelet therapy,it may become a new concept and more optimized choice for the development of interventional therapy in the future.
