Objective:To observe the clinical effect and safety of capsaicin peppermint oil emulsion(juan tong gao)in the auxiliary treatment of acute migraine,and provide an effective and safe treatment method for the clinical treatment of acute migraine.Method:From May 2019 to December 2019,72 patients with acute migraine who met the inclusion and exclusion criteria were enrolled in the Department of Neurology,the first hospital of Wuhan.They were randomly divided into the treatment group(Juan tong gao group)of 36 cases and the control group of 36 cases.The drug selection of the control group was based on the Chinese migraine prevention and treatment guidelines for patients with acute migraine,rizatriptan benzoate tablets,and the treatment group was treated with the application of Fengchi point of Juan tong gao on the basis of the treatment of the control group.Acupoint application scheme: the patient takes the sitting position,leans his head forward,fully exposes the skin around Fengchi point,and cleans the local skin,and then evenly applies Hetong cream to the skin of Fengchi point on both sides,once a day for 3-4 hours.Drug regimen: give rizatriptan benzoate tablets,Oral administration 5mg,once a day.The main outcome measures were the number of Freedom from pain patients in 2 hours and the number of patients with the most troublesome migraine related concomitant symptoms in 2 hours,The secondary outcome measures included 2hour headache relief,2-24 hour sustained freedom from pain,2-24 hour headache sustained relief,onset time,VAS score before and after treatment and TCM syndrome score.The safety was observed and the adverse reactions were recorded.In the course of treatment,there were 3 cases fall off in the treatment group and 1 case fall off in the control group.Finally,68 patients successfully completed the clinical study,including 33 cases in the treatment group(7 males and 26 females),35 cases in the control group(10 males and 25 females).Result: 1.A total of 68 patients(including 17 males and 51 females)completed the study.Before treatment,the patients in the two groups had sex,age,BMI,the history of family,aura,location of headache,nature of headache,course of headache,degree of headache,frequency of headache,accompanying symptoms,imaging examination results,VAS score before treatment and TCM symptoms There was no significant difference in syndrome score(P > 0.05).2.Efficacy: After treatment,the total effective rate of the treatment group was 81.8%,and the total effective rate of the control group was 57.1%.Treatment group was significantly better than the control group(P < 0.05).3.Main outcome measures: after treatment,the number of patients in the treatment group who had freedom from pain 2 hours after treatment and the number of patients who had the most troublesome migraine related symptoms relieved 2 hours after treatment were better than those in the control group(P < 0.05).4.The subprogramme measures:(1)The number of patients in the treatment group who had pain relief in 2 hours,headache sustained relief in 2-24 hours,sustained freedom from pain in 2-24 hours and effective time were all better than those in the control group(P < 0.05).(2)VAS score: after treatment,the VAS score of the two groups was significantly lower than that of the same group before treatment,and the score of the treatment group decreased significantly(P < 0.05).(3)TCM syndrome score: After treatment,the TCM symptom scores of the two groups were significantly improved compared with that of the same group before treatment,and the treatment group was significantly improved(P < 0.05).5.In terms of adverse reactions,there was no difference in nausea,dry mouth,dizziness and fatigue between the treatment group and the control group(P > 0.05).In terms of safety,there were 3 patients in the treatment group with mild burning sensation of skin,after the treatment of local physical cooling,the symptoms gradually disappeared.There was no difference in local and systemic reactions between the two groups(P > 0.05).Conclusion:This study preliminarily explored the clinical effect of Acupoint Application of capsaicin peppermint oil emulsion(juan tong gao)plaster on acute migraine.Compared with the simple oral western medicine treatment,combined with Acupoint Application of Juan tong gao plaster can improve the pain degree of acute migraine patients faster,relieve the accompanying symptoms of patients,shorten the starting time of pain relief,reduce the syndrome score of traditional Chinese medicine,and in the process of treatment,the adverse reactions are small,the safety is high,It can be regarded as an effective treatment method in clinical practice and is worth further study.
