Clinical Effect Observation Of Capsaicin Peppermint Oil Emulsion Combined With Drugs For The Preventive Treatment Of Migraine Without Aura

Background:Migraine is a kind of primary headache which is very common in clinic.It can be generally divided into two types: migraine with aura and migraine without aura.Among them,migraine without aura is more common,accounting for about 80% of migraine patients.Its treatment should include two stages: acute stage and intermittent stage.However,due to a variety of reasons,most of the patients generally only receive pain relief treatment in acute stage and ignore the importance ofpreventive treatment.Some data show that about one third of patients should receive preventive treatment to reduce the frequency of headache attack and the abuse of painkillers,which are the risk factors for the conversion of episodic migraine to chronic migraine.Frequent headache attacks will seriously affect the quality of life of patients,so preventive treatment can not be ignored.At present,the preventive treatment of migraine is mainly western medicine.In view of various adverse reactions such as drowsiness,dizziness,weight gain and so on,its clinical application is greatly limited.Many people turn their focus to traditional Chinese medicine.Its rich treatment methods,including traditional Chinese medicine,acupuncture,acupoint application,which are safe,reliable,convenient,and with little side effects,is beingmore and more recognized by people.The treatment of migraine with Juan tong tie in the early stagehas shown a positive effect;the foreign literature reported that capsaicin has a long-term analgesic effect,andits external preparation has been widely used in the treatment of inflammatory pain,neuropathic pain and other kinds of acute and chronic pain,and showed a good prospect of treatment.In summary,in order to improve the curative effect and optimize the treatment plan,the tutor proposed to use capsaicin peppermint oil emulsionexternal application at Feng-chi pointcombined with acupoint application therapy,it was used in the experimental preventive treatment of migraine without aura in the intermittent period,in order to provide a new treatment for the clinical.Objective:To observe the clinical effect of acupoint application of capsaicin peppermint oil emulsion combined with drugs for the preventive treatment of migraine without aura.Methods:From March 2019 to September 2019,40 migraine patients were selected from the headache clinic of the Department of Neurology of Wuhan integrated traditional Chinese and Western medicine hospital.According to certain inclusion and exclusion criteria,40 patients were recruited,which were randomly divided into two groups:the control group(20 cases)and the observation group(20 cases).The control group was given oraltopiramate alone to prevent migraine.On this basis,the observation group was applied with acupoint application of Feng-chipoint with capsaicin peppermint oil emulsion additionally.After the end of this experiment,the difference in the frequency of headache attack,duration of headache and severity of headache between two groups were compared,and the adverse reactions during this experiment were recorded.Results:In terms of headache attack frequency,compared with the baseline level,the observation group and the control group can effectively reduce the number of migraine attacks per month(P<0.01),the observation group has more obvious advantages,the difference is statistically significant(P<0.05);in terms of headache duration,compared with before treatment,the level of the two groups after treatment has decreased,the difference is statistically significant(P<0.05),the observation group's duration of headache was shorter than that of the control group,but the difference was not statistically significant(P>0.05).The VAS score of the two groups after treatment was significantly lower than that before treatment(P<0.01),and the VAS score of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).In terms of adverse reactions,3 patients in the control group had side effects of drowsiness,2 patients had side effects of fatigue and dizziness,while 4 patients in the observation group had side effects of drowsiness,2 patients reported redness and pruritus at the application site,with nosignificant difference.Conclusion:capsaicin peppermint oil emulsion acupoint application combined with drugs for Migraine without aura can effectively reduce the frequency of headache attack,shorten the duration of headache,reduce the severity of headache,and it is safe and reliable.It is worth popularizing as a treatment in clinical practice.
