The Efficacy Of LAEA With Transdermal Microneedles For Drug Delivery In Improving The Quality Of Life Of Patients With Moderate To Severe Knee Osteoarthritis

Objective:The improvement of quality of life of patients with moderate to severe knee osteoarthritis who refused surgery or had contraindications due to the indications of joint replacement was observed.Methods:Thirty patients with moderate to severe knee osteoarthritis who had indications of joint replacement but refused surgery or had contraindications were randomly divided into two groups:one group was treated with LAE A;the other group was treated with voltaren.The course of treatment was 1 year.The drug was started when the group was enrolled,stopped for 1 month after 1 month,and so on until the end of the course of treatment.The total score of WOMAC,X-ray grade and blood lipid before and after treatment were compared to evaluate the clinical effect of LAEA on moderate to severe KOA,and to analyze the relationship between MRI pathological changes,X-ray grade and blood lipid in KOA patients.Results:1.There were no significant differences in the demographic data of the two groups,the total score of WOMAC before treatment,the X-ray grade,and the level of blood lipid,which were comparable(P>0.05).2.The total score of WOMAC in both groups decreased after treatment,with statistically significant difference(P<0.05),and the score after LAEA treatment was bette than that in the voltaren group(P<0.05).3.There was no significant difference in X-ray grading between the two groups after treatment(P>0.05).4.Low-density lipoprotein cholesterol(LDL-C)concentration in LAEA group increased after treatment(P<0.05),while total cholesterol(TC),triglyceride(TG)and high-density lipoprotein cholesterol(HDL-C)concentrations were not significantly changed(P>0.05).There was no significant change in the concentration of TC,TG,HDL-C and LDL-C in the voltaren group after treatment.There was no significant change in the proportion of normal and abnormal patients in the two groups after treatment(P>0.05).5.In terms of ligament injuries,there were significant differences among different X-ray grades,and the proportion of ligament injuries in grade 4 patients was higher.There was no significant difference in MRI pathological changes between patients with hyperlipidemia(high TC or high TG)and patients with normal lipid levels(normal TC and TG)(P>0.05).Conclusion:LAEA with transdermal microneedles for drug can improve the pain and stiffness symptoms of patients with moderate to severe knee osteoarthritis and improve their quality of life.It provides a better option for patients who meet the criteria for joint replacement but refuse surgery or have contraindications to reduce pain.The mechanism may be related to the regulation of LDL metabolism.
