Objective:To investigate the relationship between venous catheterization and pulmonary thromboembolism(PTE);to investigate von Willebrand factor(vWF),vascular endothelial growth factor(VEGF),tissue plasminogen activator(t-PA),Platelet parameters(PLT,MPV,PCT,PDW),coagulation four(APTT,PT,FIB,TT),thrombus markers(D-dimer),inflammation index(WBC)caused by pulmonary thromboembolism in venous catheterization(The role of PTE)is to find early sensitive indicators of PTE formation, prevent the occurrence of PTE and monitor its development, and provide a theoretical basis for the thrombolysis of PTE to reduce the harm caused by PTE.Methods:The rabbit pulmonary artery model was made to understand the high density distribution of pulmonary artery. The paraffin specimens of the lung tissue of the experimental group were collected and HE stained to observe the occurrence and pathological changes of PTE.According to the occurrence of PTE,the relationship between venous catheterization, catheterization mode, drug permeability, indwelling time,catheter-related thrombus(CRT),catheter-related thrombus classification and PTE formation was analyzed according to the animal model.According to the staining results of HE,they were divided into embolization group,non-embolization group and blank group.According to the embolization group, the non-embolization group and the blank group, the blood indexes before and after the placement of the catheter were reviewed, and the blood indexes PLT,MPV,PCT,PDW,APTT,PT,FIB,TT,DD,WBC were placed in the venous catheter. The role in PTE.According to the embolization group,non-embolization group and blank group, all lung tissue samples were immunohistochemically stained by SP method to detect the expression of t-PA,vWF and VEGF,and the relationship between PTE formation and vascular endothelial cell injury was analyzed.Results:1.The distribution of PTE is mainly in the right lung lobe and the lower lobe.According to the left and right lung lobe, the distribution of the right lobe leaves accounted for 61.11%;the upper lobe leaves were differentiated, and the lower lobe leaves accounted for 77.78%.2.venous catheterization will lead to the formation of PTE,the incidence of PTE is 15.00%(18/120),the formation of PTE has nothing to do with the location of the catheter(the ear vein vein catheter, the external jugular vein catheter)(P>0.05),independent of drug permeability(P>0.05),independent of catheterization time(P>0.05).3.The occurrence of PTE was positively correlated with CRT classification,r=0.323,P=0.000.The more serious the CRT,the higher the incidence of PTE.4.Compared with FIB,the FIB values of the embolization group(5.528±1.630g/L)and the non-embolization group(5.304±1.983g/L)were higher than the blank group(3.320±0.992g/L),the difference was statistically significant.(P<0.05).The other blood indicators,PLT,MPV,PCT,PDW,APTT,PT,TT,D-D,WBC in the embolization group,non-embolic group and blank group were not significantly different(P>0.05).5.Compared with t-PA,the positive expression rate of embolization group was 5.56%.The positive expression rates of non-embolization group and blank group were 52.78%and 56.25%,respectively.The expression of t-PA in embolization group was lower than that in non-embolization group. In the blank group, the difference was statistically significant(P<0.05).6.Compared with vWF,the positive expression rate of embolization group was 83.33%.The positive expression rates of non-embolization group and blank group were 19.44%and 31.25%,respectively.The expression of vWF in embolization group was higher than that in non-embolization group and blank group.Statistically significant(P<0.05).7.Compared with VEGF,the positive expression rate of embolization group was 61.11%.The positive expression rates of non-embolization group and blank group were 13.89%and 6.25%,respectively.The expression of VEGF in embolization group was higher than that in non-embolization group and blank group.Statistically significant(P<0.05).Conclusions:1.The formation of CRT caused by venous catheterization can eventually lead to the formation of PTE.2.The classification of CRT is positively correlated with the formation of PTE.The more serious the CRT,the higher the incidence of PTE.3.When PTE is formed, the endothelial cells of the body are further damaged, and it is in a state of high coagulation and low fiber activity. The changes of t-PA,vWF and VEGF can be used as new ideas for thrombolysis of PTE.
