Objectives1.Master the current situation of six common chronic diseases and their combination in a community of Yangzhou.2.Study the current situation of hypertension management in this community and to analyze its possible influencing factors.3.Construct a five-color classification standard suitable for strengthening the management of hypertension key personnel in the community.Methods1.Based on the basic public health service health data of a population in a communityof Yangzhou from 2017 to 2019,the current situation study was used to describe the prevalence of six common chronic diseases and the prevalence of multiple chronic diseases,and the influencing factors were explored according to gender,age,household registration,marriage and other demographic characteristics.2.To study the prevalence,management and control of hypertension patients of different genders and ages,and to explore the influence of other chronic diseases on the management and control of hypertension.3.Based on the risk of death,a five-color classification standard for the health management of patients with hypertension was constructed.By comparing the mortality rate of hypertension combined with other chronic diseases,risk factors and population risk levels of diferent grades were determined.Results1.The total population of the study community was 63,550,among whom 10,547 had hypertension and the prevalence of hypertension was 16.60%.The initial age of hypertension was between 11 and 15 years old.The number of patients with diabetes was 3,247,and the prevalence rate was 5.11%.The initial age of diabetes was 26-30 years old.The number of patients with coronary heart disease was 384,the prevalence was 0.60%,and the age of onset was 41-45 years old.The number of stroke patients was 253,the prevalence rate was 0.40%,and the initial age of stroke was 46-50 years old.The number of patients with mental illness was 116,with a prevalence rate of 0.18%.The number of patients with malignant tumor was 190,the prevalence was 0.30%,and the initial age of the disease was 26-30 years old.The prevalence rates of diabetes,coronary heart disease,stroke,psychosis and malignant tumors are far lower than that of hypertension.Men have a higher risk of hypertension than women in the community;The risk of hypertension in non-registered population is lower than that in registered population.In the distribution of marital status,the risk of hypertension was higher among the widowed.In the distribution of medical payment methods,the risk of hypertension was higher among the self-paying group than among other groups.In terms of age distribution,the risk of hypertension in the community increased with age.2.There are multiple chronic diseases in the community population.The total number of chronic diseases in the community is 11869,accounting for 18.68%of the total population in the community.The number of people suffering from only one chronic disease is 9185,accounting for 77.39%of the total number of chronic diseases;The number of people with two chronic diseases was 2502,accounting for 21.08%;the number of people with three or more chronic diseases was 182,accounting for 1.53%.3.There is a statistically significant difference in the number of chronic diseases among the community population between gender,marital status,type of permanent population,and medical payment method(P<0.001).There were statistically significant differences in age,medical payment methods,and marital status distribution among hypertensive people of different genders in the community(P<0.001).Men have a higher risk of hypertension than women;unmarried and widowed people have a lower risk of hypertension than married people;new rural cooperative medical insurance and fully self-paying people have a lower risk of hypertension than medical insurance people;by age group The rate shows that the risk of hypertension in the community increases with age.4.The management rate of hypertension patients in community was 81.33%.From 2017 to 2020,the overall hypertension control rate of the follow-up population of hypertension patients in the community was 90.08%,much higher than that reported in similar studies.25.17%of hypertensive patients had more than one chronic disease.When combined with a chronic disease,the number of patients with diabetes was the largest(2111),with a composition ratio of 20.02%,which was significantly higher than that of other chronic diseases.When the two chronic diseases were combined,the number of people who combined "coronary heart disease+diabetes" was 73,with a composition ratio of 0.69%;the number of people who combined"stroke+diabetes" was 61,with a composition ratio of 0.58%.The combination of more than three chronic diseases is rare,only 2 people.5.Yellow risk factors were identified according to the mortality rates of people with various combinations of diseases:coronary heart disease and diabetes;Orange risk factors:stroke and psychosis.The community hypertension population was divided into five risk levels:purple,red,orange,yellow and green.The mortality rates corresponding to the five risk levels were 16.92%,14.77%,10.33%,4.15%,and 1.53,respectively,and the proportion of hypertension patients was 0.62%,0.73%,2.02%,21,81%,and 74.83%,respectively.ConclusionsThis study comprehensively describes the distribution characteristics of chronic diseases in the Shuangqiao community of Yangzhou City.It is found that more than a quarter of the people in this community suffer from at least one chronic disease,among which hypertension is the most common and has the highest prevalence rate.There are more serious people with multiple chronic diseases.Demographic factors such as gender,marital status,type of permanent population,and medical payment methods all have an impact on the prevalence of chronic diseases.Although hypertension is managed in this community,its control rate is very low.Diabetes is the most common chronic disease associated with hypertension.But it's stroke that has the greatest impact on mortality.The characteristics of mortality risk of hypertension combined with other chronic diseases were compared,and the standard of five-color classification was established,so as to realize the gradation of risk management and the controllability of population size of patients with hypertension,which is convenient for community application and implementation.
