ObjectiveTo explore the risk factors of malignant tumor with venous thromboembolism,and to analyze the clinical features and prognosis of malignant tumor with venous thromboembolism,and provide clinical theoretical basis for early clinical prevention and early diagnosis of tumor-related venous thromboembolism.MethodsA total of 1000 patients with malignant tumors admitted to the Department of Oncology in the First Affiliated Hospital of Xinxiang Medical University from January 2015 to July 2019 were selected as subjects.Among them,88 patients had venous thromboembolism.The gender,age,BMI,combined disease,tumor type,tumor stage,tumor treatment history,and thrombosis were recorded.General information on the condition,history of surgery,use of hormones,central venous catheterization,bed rest,infection,use of hemostatic drugs,blood transfusion,etc.,Laboratory parameters such as white blood count,platelet count,hemoglobin,antinuclear antibody,blood homocysteine,D-dimer,fibrinogen,plasma prothrombin time,activated partial thromboplastin time,and plasma thrombin time were measured.Univariate and unconditional multivariate logistic regression were used to analyze malignant tumors risk factors for venous thromboembolism.Results1.Patients with malignant tumors complicated with venous thromboembolism compared with patients with simple malignant tumors,found in age,BMI,white blood cell count,platelet count,hemoglobin,bed rest,hormone application,antinuclear antibody,surgical history,central venous catheter,blood homocysteine on ammonia,the difference was statistically significant(P<0.05).2.Patients with malignant tumor complicated with venous thromboembolism in 58 cases of deep venous thrombosis,12 cases of pulmonary embolism,18 cases of deep vein thrombosis + pulmonary embolism,10 cases involving upper limbs and 66 cases of lower limbs,mainly manifested as limb swelling,pain,cough after coughing,etc.,after anticoagulant therapy,50 cases were improved,28 cases were ineffective,and 10 cases died.3.Unconditional multivariate logistic regression analysis found that BMI(OR=5.335),white blood cell count(OR=2.341),platelet count(OR=7.215),and hemoglobin(OR=4.325)were independent risk factors for malignant tumors with VTE.Conclusion1.The incidence of malignant tumors and VTE is high,which is caused by multi-factor synergy.The main factors include age,BMI,white blood cell count,platelet count,hemoglobin,bed rest,hormone application,antinuclear antibody,surgical history,central venous Tube,blood homocysteine,in which BMI,white blood cell count,platelet count,hemoglobin are independent risk factors.2.Patients with malignant tumors complicated with venous thromboembolism have significantly more thrombosis than upper limb thrombosis.The main symptoms are limb swelling,pain,cough,cough,etc.Anticoagulant therapy can effectively improve the prognosis of patients.3.The prevention and treatment of malignant tumor with venous thromboembolism needs further regulation.High-risk patients should actively take preventive measures during the perioperative period to reduce the occurrence of venous thromboembolism.Active comprehensive treatment should be given to patients with venous thromboembolism.
