ObjectiveTo study whether they had significant differences in index of thromboelastography(TEG) of patients with pure deep vein thrombosis(DVT) and patients with DVT and pulmonary embolism(PE).The index of TEG is used to assess the possibility of occurrence of PE in patients with DVT; meanwhile, it was explored whether TEG index and blood coagulation index had correlation in VTE, and whether they could be comprehensively applied to VTE diagnosis.MethodsWe collected the patients who consulted in Hunan Provincial People’s Hospital from March, 2015 to February, 2016. In strict accordance with selection criteria 120 patients were finally chosen, they were divided into 3 groups by adopting the principle of match grouping of which there were 40 cases of DVT patients at newly acute stage and 40 cases of the diagonosis of DVT and PE at newly acute stage in treatment group(PE group for short); there were 40 cases of patients in match group. In treatment group, patients were examined by utilizing colorDoppler ultrasound of lower limbs and computed tomography pulmonary angiography(CTPA). In each group, patients were all examined by utilizing TEG and coagulation function.ResultsThe difference of TEG index(reaction time(R),kinetics time(K), maximum amplitude(MA)and angle(α)) between PE group and DVT group was statistically significant(P < 0.05).Besides, prothrombin time(PT),international normalized ratio(INR), activated partial thromboplastin time(APTT), thrombin time(TT),fibrinogen(FIB),D dimer and R had a large correlation(r was respectively 0.585, 0.565, 0.675,-0.341,-0.775,-0.708,and p<0.05). The value of PT, INR, APTT, FIB, D dimer and K a large correlation(r was respectively 0.309, 0.540, 0.601,-0.574,-0.734, and p<0.05). The value of PT, INR, APTT, D dimer and α had a large correlation(r was respectively-0.430,-0.475,-0.351, 0.364, and p<0.05). The value of INR, APTT, D dimer and MA had a large correlation(r was respectively-0.466,-0.384, 0.353, and p<0.05).Conclusion(1) TEG may be used to evaluate and distinguish the significant of the occurrence of PE in patients with DVT.(2) There was a significant correlation between the index ofTEG and coagulation function test in diagnosis of VTE.It can play a complementary role with combined use of TEG and coagulation function in the diagnosis of VTE.The TEG examination was diagnosed the risk of bleeding may be superior to the coagulation function.
