Background: In recent years,with the improvement of people’s awareness of skin diseases,skin aging has become a topic of increasing concern to the general public.There is a growing demand for effective interventions that can prevent or postpone the symptoms of skin aging.At the same time,with the continuous improvement of the level of medical cosmetic technology,the number of people receiving medical cosmetic treatment in hospitals has increased significantly.Skin aging is a complex process,which is the change of skin elasticity and function caused by many factors.The causes of skin aging can be divided into endogenous factors(age,etc.)and exogenous factors(ultraviolet radiation,etc.),the latter is called skin photoaging.Its clinical manifestations are pigmentation,telangiectasia,large pores,wrinkles,skin relaxation,rough,solar keratosis,precancerous lesions and malignant tumors.Intense pulsed light is a multi-wavelength incoherent light with a wavelength range between 500nm-1200 nm.It is a kind of intense light emitted by pulses.In clinical treatment,different wavelengths of intense pulsed light can be obtained by selecting different filters to meet different treatment requirements.Because of its good therapeutic effect and safe and simple clinical application,it is widely used in medical cosmetology and skin disease treatment.In this study,M22 King Laser Photon Therapy instrument of American Science and Medicine Company was used to treat pigmentation,erythema and telangiectasia caused by light aging.by selecting appropriate intense pulsed light parameters(wavelength,pulse width,energy,etc.)to target different color groups(water,melanin,hemoglobin),so as to achieve the cosmetic effect of removing freckle and red blood thread.Objective: Through the self-control study before and after IPL treatment,ANTERA 3D camera was used to evaluate the improvement of pigmentation and telangiectasia caused by intense pulsed light treatment of facial skin photoaging.At the same time,the main postoperative adverse reactions were observed,the safety of IPL equipment was evaluated,and the satisfaction of patients was investigated.Methods: from January 2019 to January 2020,45 female patients with facial skin aging problems and requiring non-operative treatment were selected from the Department of Dermatology of the second Hospital of Dalian Medical University,aged from 25 to 50 years old,with an average of 39.60 ±6.14 years.The skin type was fitzpatrick II-IV.Among them,there were 42 patients with abnormal pigmentation and 20 patients with erythema and telangiectasis.there were 17 cases with abnormal pigmentation and erythema and telangiectasia,and 45 cases showed varying degrees of fine wrinkles,large pores,skin relaxation and rough touch.These patients were treated with IPL for a total of 4 times,with an interval of about 4 weeks.The imaging data were retained before each treatment,4 weeks after each treatment and 3 months after the last treatment.The curative effect was observed according to the comparison of skin lesions before and after treatment,and the clinical photoaging symptoms of the patients were scored and the degree of improvement was evaluated.The final data were analyzed by statistical software SPSS 26.0 to determine whether the treatment effect was statistically significant.At the same time,the adverse reactions after treatment were observed and the satisfaction of patients was investigated.Results: a total of 45 patients were included in this study,and effective data were obtained.1.The physician’s subjective score of abnormal pigmentation symptoms before and after treatment in the pigmentation group decreased from 2.80 ± 0.57 points to 2.13 ± 0.61 points(P<0.05).The subjective score of erythema and capillary dilatation symptoms before and after treatment in the erythema group decreased from 1.88 ± 0.43 points to 1.28 ± 0.41 points(P<0.05).2.The self-assessment score of the individual symptoms of photoaging before and after treatment in the pigmentation group decreased from 6.298 ± 0.733 to 3.536 ± 0.550(P<0.05);the self-assessment score of the individual symptoms of photoaging before and after treatment in the erythema group From 5.475 ± 1.186 points to 2.975 ± 0.716 points(P<0.05).3.It was concluded through non-invasive skin testing that the average level of melanin in patients with pigmentation group decreased from 100% before treatment to 80.24% ± 2.34%(left side)(P<0.05),79.90% at 4 weeks after the last treatment.± 2.29%(right)(P<0.05);decreased to 78.06% ± 2.94%(left)3 months after the last treatment(P<0.05),77.64% ± 2.74%(right)(P<0.05).In the erythema group,the average level of cheek hemoglobin decreased from 100% before treatment to 78.61% ± 2.12%(left side)(P<0.05),79.26% ± 1.75%(right side)(P<0.05).The average hemoglobin level decreased to 76.55% ± 2.76%(left)(P<0.05)and 76.76% ± 2.11%(right)(P<0.05)3 months after the last treatment.4.During the whole treatment process,the main adverse reactions were transient pain and burning sensation during operation.45 patients indicated that the discomfort was within the tolerable range.After the end of treatment,they could be completely relieved by ice application.Local skin crusting,swelling and other adverse reactions occurred after the operation.5.Four weeks after the last treatment,the total satisfaction of the patients in the color precipitation group was 80.99%,and the total satisfaction of the patients in the erythema group was 90.005% at the end of the last treatment.Conclusion:1.Intense pulsed light has an obvious effect on improving the photoaging of facial skin.2.Intense pulsed light can achieve the purpose of improving symptoms such as abnormal pigmentation and telangiectasia caused by skin photoaging.3.Most of the adverse reactions after intense pulsed light therapy are transient and reversible,and they have been widely used in clinical to treat a variety of skin cosmetic problems.
