Objective:Based on the theory of acupoint sensitization,this subject explores the distribution of pain sensitivity in the relevant nerve segments of the chest and back in patients with stable angina pectoris of coronary heart disease.Acupuncture is used to stimulate the acupoints on the sensitized and non-sensitized areas of the same nerve segment.By comparing the quality of life of patients ? the improvement of clinical symptoms,preliminary exploration of acupuncture methods suitable for stable angina pectoris of coronary heart disease,to provide a new reference method for clinical acupuncture to select points.Methods: 120 patients with stable angina pectoris of coronary heart disease were selected according to the criteria of nanoarray and randomly divided into:(1)conventional acupuncture group(control group),(2)sensitized area combined with non-sensitive acupuncture group(test group 1)),(3)Sensitive area acupuncture group(test group 2),(4)non-sensitive acupuncture group(test group 3),30 cases in each group.First of all,the sensitized areas of the nerve segment belonging to the heart and the adjacent chest and back are detected,and the pain threshold is measured by von frey electronic pain tester,which is measured 3 times,and the average value is taken.The control group selected Neiguan,Yinxi,Danzhong,and Ximen points;the experimental group 1 selected Yinxi,Tongli,Shenmen and the lowest point of the pain area of the pain area;the experimental group 2 selected the relevant nerve segments Acupuncture at the lowest point of the pain area in the pain area;the experimental group 3,three points of Yin,Tongli,and Shenmen of the forearm of the same nerve segment were taken.The above three groups all used lightstimulation techniques of lifting,twisting,relieving and relieving.The acupuncture was performed 3 times a week.The needle was kept for 30 minutes after the qi,and the treatment lasted for 3 weeks.Record the Seattle Angina Scale(SAQ),TCM symptom score score,pain threshold,ECG index before and after treatment,and statistically analyze the relevant data.Results:1.120 patients with coronary heart disease and angina pectoris have 256 sensitization points / areas on the chest and back,and there are 1-2 more tender spots;distribution frequency: left chest> right chest> sternum stem> left back> right back;more It is on the route of Renmai,Foot Shaoyin Kidney Meridian,Foot Yangming Stomach Meridian,Hand Jueyin Yin Pericardium Meridian.2.The pain threshold of the four groups of patients after acupuncture treatment was higher than before(P<0.05),and the pain threshold of the observation group was more significant than that of the other three groups(P<0.05),all with statistical differences;the pain threshold of test group 2 was more significant than that of test group 3(P<0.05),with statistical differences.3.Seattle Angina Scale score: Seattle scale scores were compared among different groups.The comparisons between the groups of activity limitation,angina pectoris stable state,angina pectoris attack,treatment satisfaction,and disease awareness were all P<0.01;then make a pairwise comparison between the groups,the results have the observation group scores of each dimension compared with the other three groups are statistically significant,all P<0.05,indicating that the observation group scores in each dimension are better than the other group;test group 2 has statistical significance in improving physical activity and disease awareness compared with test group 3,both of which are P <0.05,indicating that test group 2 is better than test group 3 in physical limitation and disease recognition.4.TCM syndromes: Compared with patients before and after treatment,the total effective rate of TCM syndromes in the observation group is 90%,the total effective rate in the experimental group 1 is 63.33%,the total effective rate in the experimental group 2 is43.33%,the total effective rate in the experimental group 3 is 43.33%,four groups The patients all improved to varying degrees,and the total effective rate of the observation group was higher than that of the other three groups,with statistical significance(P<0.05).5.The effect of electrocardiogram: The rank sum test between multiple groups was used to compare each other.The total effective rate of the observation group was 86.67%,the total effective rate of the test group 1 was 66.67%,the total effective rate of the test group 2 was 50%,and the total effective rate of the test group 3 was 43.33%.The total effective rate of the observation group was higher than that of the other three groups,with statistical significance(P<0.05).Conclusion:1.Stable angina pectoris sensitization of coronary heart disease mostly occurs in the body surface area dominated by the same segment or adjacent spinal nerve segments,and the susceptible diffusion of sensitization according to the condition of the disease is distributed in different range shapes.2.The four acupuncture methods have a certain degree of efficacy in the treatment of stable angina pectoris,and the analysis results show that:(1)Comprehensive acupuncture points on the chest and back combined with upper limbs improves TCM symptoms and restricts the activity of patients with single chest or upper limbs Situation,angina pectoris stability and seizures,body surface pain threshold,electrocardiogram effect is better;(2)Sensitized acupuncture point is better than non-sensitized point acupuncture in improving patient surface pain and activity limitation;(3)The prescription of acupuncture points is particularly important in clinical treatment.The combination of acupoint sensitization and acupoints can improve the therapeutic effect of stable angina pectoris.Therefore,in the clinical selection of acupoints,related pain-sensitive acupoints can be used to treat the disease on the basis of comprehensive compatibility.
