ObjectiveThe purpose of this study is to investigate and compare the effects of hydrolyzed lactose milk and soymilk on bone mineral density(BMD)and bone metabolism in postmenopausal women in order to provide a theoretical basis for the prevention of postmenopausal osteoporosis from rational diet and scientific supplement.Methods1.In this study, many postmenopausal women aged 45-65 were recruited from various communities in Xiangfan city. All subjects included in the study was aged 45-65,naturally menopausal for more than one year and the diagnosis value of BMD(-2.5<T≤1).All of the subjects were excluded from the serious disease of digestive system, liver disease, diabetes, hyperthyroidism, hyperparathyroidism, connective tissue disease or other factors affect bone metabolism.Subjects never used or use no more than 3 months of drugs, which affecting bone metabolism, such as glucocorticoid, estrogen and thyroid hormones, chemotherapy drugs, vitamin D,calcium and so on.Finally,a total of 94 eligible postmenopausal women without osteoporosis were included from the recruitment of Xiangfan community volunteers.2.Simple questionnaires had been designed to investigate and analyze the BMD and risk fators assoeiated with osteoporosis in postmenopausal women. The study was designed as a non-blind,randomized controlled intervention study. The subjects were randomly divided into three groups for 6 months of intervention trials: the hydrolyzed lactose milk group, the soymilk group and the control group. The women in the hydrolyzed lactose milk group drank 500 m L of hydrolyzed lactose milk daily during the6-month intervention. The women in the soymilk group drank 500 m L of soy milk per day.The control group was not given a placebo. The BMD of lumbar spine was determined using DXA in three groups before and after the experiment, respectively.Serum glucose, lipid level, beta crosslaps(β-CTx),procollagen I N-terminal propeptide(PINP),parathyroid hormone(PTH)and N-MID osteocalcin(N-MID-OT)of the 101 eligible postmenopausal women in the hydrolyzed lactose milk, soymilk and control groups were examined before and after intervention.Results:1.After 6-month experiment, the BMD of the lumbar spine among the three groups were decreased, while the BMD of the control group decreased significantly(P<0.05).The drop of BMD in the hydrolyzed lactose milk group and soymilk group in 6 months was significantly lower than that in control group(P<0.05),and the T value in the range of decline in the hydrolyzed lactose milk group was significantly lower than that of soymilk group(P<0.05).2.In this study, the bone metabolic markers in subjects of early menopausal women were at a high level.Comparison of bone metabolic markers before and after intervention, the serum levels of β-CTx,P1 NP and N-MID-OT have different degree increased level than the baseline status. The level of serum N-MID-OT in three groups significantly increased compared with the baseline level(P<0.01).Moreover,the level of serum β-CTx in the control group was significantly higher than baseline(P<0.05),while the level of PINP in the hydrolyzed lactose milk group and the soymilk group was significantly increased when compared with that in the baseline(P<0.01).In the hydrolyzed lactose milk group and the control group, the PTH levels were higher than the baseline, while the PTH levels in the soymilk group decreased slightly(P>0.05).However,after 6 months of drinking hydrolyzed lactose milk or soymilk for 500 m L,there was no significant difference in bone metabolism index between the three groups(P>0.05).Conclusion:1.Our results suggested that six months of drinking hydrolyzed lactose milk or soymilk for 500 m L daily can significantly reduce bone loss in postmenopausal women.2.In our study, early menopause of women are at an active period of bone turnover.Six months of drinking hydrolyzed lactose milk or soymilk for 500 m L/day may promote the activity of osteoblasts, inhibit osteoclast bone resorption and bone transformation. However,a significant protective effect of long-term research trials on BMD and bone metabolism may also need to be further observed.
