Objective: This study retrospectively analyzes the clinical data of postmenopausal women hospitalized in Affiliated Hospital of Guilin Medical University,and explored the correlation and clinical significance of bone mineral density(BMD)and biochemical markers of bone metabolism in postmenopausal women.Methods : The clinical data of 292 postmenopausal women admitted to the Department of Geriatrics and Rheumatology of the Affiliated Hospital of Guilin Medical College from October 2017 to December 2018 were retrospectively analyzed.Collect all clinical indicators of all subjects: lumbar vertebrae L1-4 BMD,Age,Body Mass Index(BMI),?-CrossLaps(?-CTX),Total-terminal Propeptide of Type I Procollagen(t-PINP),N-terminal Osteocalcin(N-MID),Alkaline Phosphatase(ALP),25-OH-VD,Hemoglobin(HGB),Platelet(PLT),Calcium(Ca),Phosphorus(P),Copper(Cu),Triglycerides(TG),Cholesterol(CHOL),High-density Lipoprotein Cholesterol,(HDLc),Low-density Lipoprotein Cholesterol(LDLc),ApolipoproteinA(Apo-A),polipoproteinB(Apo-B).According to the bone mineral density T value,the patients were divided into three groups,namely the normal bone mass group(T value ?-1.0);the bone mass reduction group(-2.5 < T value <-1.0);the osteoporosis group(T value ?-2.5).Statistical analysis was performed using SPSS 24.0 software to compare the differences in biochemical markers of bone metabolism between the three groups,and to analyze the correlation and clinical significance of bone mineral density and biochemical markers of bone metabolism.Results:1.Comparison of general clinical data of the three groups of patients: Compared with the normal bone mass group,the age of osteoporosis group increased,BMI and BMD decreased in osteoporosis group and bone mass reduction group,the difference was statistically significant(P<0.05).Compared with the bone mass reduction group,the age of the osteoporosis group increased,the BMI and BMD of the osteoporosis group decreased,and the difference was statistically significant(P<0.05).2.Comparison of bone turnover markers and bone metabolism regulating hormones in three groups of patients: Compared with the normal bone mass group,?-CTX,t-PINP,N-MID,ALP increased,and 25-OH-VD decreased in the osteoporosis group,the difference was statistically significant(P<0.05).Compared with the bone mass reduction group,the ALP increased and the 25-OH-VD decreased in the osteoporosis group,the difference was statistically significant(P<0.05).3.Comparison of general biochemical indicators in three groups of patients: there were no significant differences in P,Cu,TG,CHOL,HDLc,LDLc and APO-A between the three groups(P>0.05).Compared with the normal bone mass group,the HGB,Ca,and APO-B in the osteoporosis group decreased,the PLT in the osteoporosis group increased,and the APO-B decreased in the bone mass reduction group,the difference was statistically significant(P<0.05);Compared with the bone loss group,the PLT of the osteoporosis group increased,the difference was statistically significant(P<0.05).4.Correlation analysis: BMI,25-OH-VD,HGB,Ca were positively correlated with BMD,and the correlation coefficients were 0.283(P<0.05),0.208(P<0.05),0.164(P<0.05),and 0.179(P<0.05);?-CTX,t-PINP,N-MID,ALP,age,PLT and BMD were negatively correlated,and the correlation coefficients were-0.205(P<0.05),-0.167(P<0.05),and-0.212(P< 0.05),-0.212(P<0.05),-0.234(P<0.05),-0.212(P<0.05).Partial correlation analysis after controlling age and BMI found that the correlation between BMD and ?-CTX,t-PINP,ALP,N-MID,PLT,25-OH-VD,Ca still existed.There was a negative correlation between 25-OH-VD and ?-CTX and PLT(P<0.05).PLT was positively correlated with ?-CTX and ALP,and there was a positive correlation between each conversion marker(P<0.05).Conclusions: 1.Bone turnover markers,vitamin D,and calcium were significantly associated with bone mineral density in postmenopausal women.2.Platelets are associated with bone mineral density and bone turnover markers and may have a role in bone metabolism.3.The combined detection of biochemical markers of bone metabolism and lumbar spine bone mineral density can reflect the bone metabolism status of postmenopausal women,and provide clinical treatment guidance for postmenopausal women with abnormal bone mass.
