Purpose: To investigate the expression of soluble programmed death ligand(sPD-L1)in the blood plasma of patients with hypopharyngeal squamous cell carcinoma during radiotherapy,and explore its clinical significance.Methods:A total of 50 patients with hypopharyngeal squamous cell carcinoma diagnosed by histopathology in in Shandong Cancer hospital between January 2017 to December 2018 were enrolled.Patients were treated with radical IMRT with or without single-agent cisplatin.12 samples of healthy subjects matching in sex and age from the Health care Center of the hospital were also studied as control.The expression of sPD-L1 in plasma of patients with hypopharyngeal squamous cell carcinoma before and after radiotherapy(at least 20 times of radiotherapy)and healthy volunteers was measured using ELISA kit.And to analyze its correlation with clinical pathological features and the volume changes of primary tumor and metastatic lymph node.Data were tested and analyzed with SPSS 17.0 software.Results : The average level of sPD-L1 in blood plasma of patients with hypopharyngeal squamous cell carcinoma before and after radiotherapy were(57.34±30.81pg/ml)and(48.87±27.50pg/ml),respectively,which was significantly higher than that of healthy people(31.68±11.21pg/ml)(P=0.006?P=0.003).The correlation analysis showed that sPD-L1 level was significantly correlated with T stage before radiotherapy(r=0.332,P=0.029).The expression of sPD-L1 had no remarkable correlation with other clinicopathological features(P>0.05).The linear regression analysis showed that there was a significant positive correlation between the degree of dynamic change of sPD-L1 and the degree of change of primary tumor and metastatic lymph node before and after radiotherapy(both P<0.01).Conclusion:Inhibitors of the PD-1/PD-L1 axis for HNSCC may be entering anew era.The plasma level of s PD-L1 was higher in hypopharyngeal squamous cell carcinoma patients than that in healthy people,which was related to the T stage.And the variation of sPD-L1 had inherent relationship with tumor burden before and after radiotherapy.Our results indicated that sPD-L1 was expected to become a new indicator for predicting the efficacy and prognosis of individualized cancer treatment.
