Objective:To detect the expression of programmed death-ligand receptor-1(PD-1)and programmed cell death ligand-1(PD-L1)in head and neck squamous cell carcinoma(HNSCC),and to explore the role of PD-1/PD-L1 in HNSCC;To detect the expression level of soluble PD-L1(sPD-L1),to analyze the correlation with the expression level of PD-1/PD-L1 in tumor tissue,and to compare the dynamic changes of sPD-L1 expression level after treatment,to explore its significance;To analyze the relationship between the expression level of PD-L1 and PD-1 in HNSCC tissue and clinicopathological features;To explore the effect of induction chemotherapy on the expression level of PD-L1 and PD-1 in HNSCC tissue.Methods:62 cases of head and neck squamous cell carcinoma were divided into direct operation group(n=32)and induction chemotherapy followed by operation group(n=30).The expression levels of PD-L1 and PD-1 in postoperative HNSCC tissues were detected by immunohistochemical staining,and compared with the expression levels of PD-L1 and PD-1 in normal tissues adjacent to cancer,to explore the role of PD-1/PD-L1 in HNSCC at tissue level.The expression of sPD-L1 in peripheral blood was detected by enzyme-linked immunosorbent assay(ELISA)before and after treatment.The correlation between the expression of sPD-L1 before treatment and the expression of PD-1/PD-L1 in tumor tissue was analyzed,and the dynamic changes of sPD-L1 expression after induction chemotherapy and/or operation were compared to explore its significance.The clinical data of 62 patients were retrospectively analyzed,combined with the expression levels of PD-L1 and PD-1 in HNSCC tissues,the relationship between the expression levels of PD-L1 and PD-1 in HNSCC tissues and clinicopathological features was analyzed,and the differences of PD-L1 and PD-1 expressions in HNSCC tissues of the two groups were compared to explore the effect of induction chemotherapy on the expression levels of PD-L1 and PD-1 in HNSCC tissues.Results:The positive rates of PD-1 and PDL1 in HNSCC tissues were higher than those in adjacent normal tissues(P<0.01);There was no significant correlation between the serum sPD-L1 values of HNSCC patients and the expression of PD-1 and PD-L1 in HNSCC tissues(P=0.280,0.465).There was no significant change in the expression of sPD-L1 before and after induction chemotherapy in HNSCC patients,but the expression level of sPD-L1 in HNSCC patients after operation was lower than that before operation(P<0.01).The expression levels of PD-1 and PD-L1 in HNSCC tissues were correlated with clinical stage and lymph node metastasis(P<0.01),while induction chemotherapy had no significant effect on the expression levels of PD-1 and PD-L1 in HNSCC tissues(P=0.829,0.840).Conclusion:PD-1 and PDL1 are highly expressed in HNSCC,and the expression levels are related to clinical stage and lymph node metastasis.PD-1 and PD-L1 may be the targets of immunotherapy for HNSCC,but there is no significant correlation between serum sPD-L1 and the expression of PD-1 and PD-L1 in HNSCC before treatment.The expression level of sPD-L1 in HNSCC patients decreased after operation,which is an important biomarker of HNSCC,and its expression level may play an important role in evaluating the efficacy of HNSCC patients.Induction chemotherapy has no significant effect on the expression levels of PD-1 and PD-L1 in HNSCC tissues.
