Diabetic foot disease is one of the most serious complications of diabetes.It manifests as wounds,ulcers,ulcers,and amputations of the feet.It requires high cost and long treatment time.It is a clinically difficult problem.Its treatment difficulties are mainly based on the following two aspects:First,the pathogenic mechanism of diabetic foot disease is complex.The occurrence of diabetic foot disease is the result of the combined action of mechanisms such as peripheral vascular disease,peripheral neuropathy,and diabetic hyperglycemia in the late stage of diabetes.Because there is no special treatment,the current industry's treatment plan for "poetry controversy" can reach a consensus:the use of negative pressure sealing and drainage of foot ulcers after debridement can effectively improve the speed of wound healing.Even so,the treatment of diabetic foot disease often takes more than 1 to 3 months.Although there are currently negative pressure instruments available on the market for home treatment,the lack of professional monitoring of the instrument often leads to treatment failure.Second,the control of diabetic foot disease focuses on prevention.Studies have shown that after comprehensive prevention measures are taken,the costs associated with diabetic foot disease can be reduced by more than 60%.Based on the current medical reality in China,patients cannot obtain comprehensive guidance and lack an effective management system.Based on the above two considerations,we have done two things:First,relying on big data technology,we developed an internet-based diabetic foot management APP platform and established a three-dimensional diabetes foot management system.The platform has the following characteristics:1.The platform establishes the electronic archives of patients,runs through the concept of five carts for the management of diabetes,and tailor-makes individualized treatment and treatment plans for patients based on their basic diseases,diet and blood sugar control.2.The platform has established channels of doctor-patient communication,doctor-doctor consultation and referral,which effectively promotes the follow-up of patients and the referral and connection between upper and lower hospitals,making the treatment of diabetic foot seamless.3.The platform has the cloud data integration function,which enables doctors to conveniently extract relevant medical history and data,and provides powerful power for guiding clinical practice and scientific research.4.The platform can link portable negative pressure and other telemedicine devices and monitor them,making the in-home treatment of diabetic foot a reality.Secondly,based on our own clinical practice,we summarized the shortcomings of the existing closed negative pressure drainage instrument and improved it.This paper designs a portable negative pressure drainage device with small size,easy to use and can be linked to the sugar foot management APP platform.After initial hospitalization,patients can be treated at home with this device,which greatly reduces the length of hospitalization.We designed a prospective randomized case-control trial to verify the effectiveness of the portable negative pressure treatment model based on Internet+in improving the healing rate of podiatric foot,preventing the recurrence of high-risk foot,and improving patient compliance.Objective:to evaluate the effect of "Internet+ based portable negative pressure treatment model" on improving the treatment effect of sugar foot and the effect of high risk foot prevention management.Methods:41 patients were randomly selected from the outpatient department,including 14 patients with diabetic podiatrics and 27 patients with high-risk podiatrics,so they were divided into two subgroups.In the podiatric group,the experimental group was treated with the portable negative pressure treatment mode based on Internet+,while the control group was treated with routine debridement,infection control and negative pressure in hospital.The wound healing rate and infection control of the two groups were evaluated at the end of treatment.In the high-risk foot test group,the sugar foot management APP platform based on Internet+was applied for comprehensive and three-dimensional management.The control group was instructed to go back to the clinic after regular outpatient education.After 1-3 months,observe the difference between the two groups in the development of diabetic foot disease,review compliance rate.Results:after 1-3 months of treatment,the experimental group was significantly better than the control group in terms of the size of podiatric wounds and the control of wound infection,with statistically significant differences(P<0.05).The experimental group of patients with high-risk foot group was significantly better than the control group in whether they could be regularly reviewed and properly nursed at home(P<0.05).There was no statistically significant difference between the two groups in whether or not they changed to diabetic foot wounds(P>0.05).Conclusion:the portable negative pressure treatment model based on Internet+can effectively improve the wound treatment effect of diabetic podiatrist and improve the treatment compliance of high-risk podiatrist.
