[Objective]The effects of intermittent pneumatic pressure therapy combined with ice compress and ice compression on improving swelling, pain and sleep of affected limbs were compared and evaluated.[Methods]In this study,60 patients with closed tibiofibular fractures in Liyang Branch of Jiangsu Provincial people’s Hospital were collected.30 patients in the observation group were treated with intermittent pneumatic pressure therapy combined with ice compress to reduce swelling, while 30 patients in the control group were treated with simple ice compress to reduce swelling. In the control group,PVC biological ice bag was used to reduce swelling, that is,triple ice bag was used to compress ice in the most swollen part of the affected limb for 30 minutes, removed for 1 hour, then frozen for 30 minutes, and then removed for 1 hour. This circulates until the day before the operation.Three ice bags should be replaced from the refrigerator each time the ice is applied. The observation group was treated with intermittent treatment on the basis of the control group. The treatment cycle is from admission to the day before operation, the treatment time is 2 times a day,one hour each time, once in the morning and once in the afternoon. The difference of leg swelling circumference, the score of leg swelling score, the pain score and the sleep quality of the patients were compared at different time points.[Results]1 the difference of swelling circumference between the two groups before operation was compared with that of the control group. There was no significant difference between the two groups on the first day and the second day of admission(P>0.05).From the third day to the fifth day of admission, the difference of swelling circumference between the observation group and the control group decreased from 5.91±0.80 to 2.25±0.57,the difference of swelling circumference between the two groups decreased from 5.91±0.80 to 2.25±0.57,and there was no significant difference between the two groups on the first day and the second day of admission(P>0.05).In the control group, the difference of swelling circumference decreased from 6.13±1.18 to 3.23±0.76.The difference of swelling circumference of the affected limbs in the observation group was better than that in the control group, and the difference was statistically significant(P<0.05).Compared with the control group, the circumference difference of the observation group decreased from 6.03±0.88 to 2.25±0.57 from the second day to the fifth day,the difference was statistically significant(P<0.05),and the circumference difference from the second day to the fifth day of admission in the control group decreased from 6.27±0.99 to 3.23±0.76,the difference was statistically significant(P<0.05).2 There was no significant difference in the score of swelling degree between the two groups on the first day and the second day after admission(P>0.05).From the third day to the fifth day of admission, the score of swelling degree of the affected limb in the observation group decreased from 3.35±0.14 to 2.53±0.15,and that in the control group decreased from 4.25 ± 1.18 to 3.02±0.11.the score of swelling in the observation group was better than that in the control group. The difference was statistically significant(P<0.05).Compared with the control group, the swelling score of the observation group from the second day to the fifth day was 4.50±0.5%.13 decreased to 2.53±0.15,the difference was statistically significant(P<0.05).The swelling degree score of the control group decreased from 4.66±0.07 to 3.02±0.11 from the second day to the fifth day of admission, and the difference was statistically significant(P<0.05).3 comparison of pain relief between the two groups before operation, there was no significant difference in(Face Rating Scale,FRS)score between the two groups on the first day of admission between the observation group and the control group, and there was no significant difference in the score of facial expression pain between the two groups on the first day of admission. From the second day to the fifth day of admission, the pain score of the observation group decreased from 6.49±0.58 to 2.15±0.57,and that of the control group decreased from 7.52±0.43 to 3.37±0.76.The FRS score of the observation group was lower than that of the control group(P<0.05).It is statistically significant. There was significant difference between the observation group and the control group from the first day of admission to the fifth day of admission(P<0.05),and the difference between the first day of admission and the second day of admission to the fifth day of admission(P<0.05),while that of the control group was significantly different from the second day of admission to the fifth day of admission(P<0.05).4 comparison of sleep quality between the two groups before operation, there was no significant difference in Pittsburgh sleep quality(Pittsburgh Sleep Quality Index,PSQI between the two groups on the first day of admission(P=0.851>0.05).There was no significant difference in PSQI score between the observation group and the control group on the second day of admission(P=0.614>0.05).From the third day of admission, the PSQI scores of the control group and the observation group decreased, the difference was statistically significant. However, from the first day of admission to the week of admission, the PSQI scores of the control group were all.It was higher than that of the observation group, indicating that the sleep quality of the observation group was better than that of the control group. The PSQI score>7 was used as the critical state of sleep disorder, the PSQI score of the observation group was more than 7 from the first day to the fourth day of admission, which indicated that the patients had different degrees of sleep disorder within four days of admission, and the PSQI score of the control group was more than 7 points from admission to the sixth day, indicating that the patients had different degrees of sleep disorder within six days of admission.[Conclusion]In this study, the study of relieving swelling in the observation group and the control group was satisfactory. The intermittent pneumatic pressure in the observation group could effectively relieve the swelling of the affected limb, relieve the pain of the affected limb, and improve the sleep quality of the inpatients. The effect and advantages were more obvious than the simple routine nursing.
