| 【Objectives】1 Construction of the "early swelling kinesio taping and binding scheme for patients with closed fracture of the middle and lower third of the tibia";2 To explore the effect and clinical application value of kinesio taping technology in patients with orthopedic traumatic early swelling,and provide a scientific and operational basis for the application of kinesio taping technology in orthopedic traumatic early swelling.【Methods】1 The first part of the program construction: through consulting relevant authoritative literature and books combined with clinical practice to extract scientific and reasonable interventions,the first draft of the "Early Swelling kinesio taping Binding Program for Patients with Closed Tibia Middle and Lower 1/3 Fractures" was preliminarily constructed,and the expert group meeting demonstration method was used to demonstrate and revise the plan,sort out expert opinions,revise and improve the kinesio taping scheme and conduct pre-experiments to form the final draft.2 The second part of program application: establish a multidisciplinary team,and select 48 patients with closed fracture of the middle and lower 1/3 segment of the tibia who met the inclusion criteria from October 2021 to September 2022 in the Department of Bone and Trauma,A Class III Class A hospital in Yunnan Province.The study subjects were randomly divided into test group(24 cases)and control group(24 cases)by random number table method.The control group was treated with routine detumescence treatment,and the test group was intervened with kinesio taping scheme based on routine detumescence treatment.The main observation indexes were the swelling value of the lower leg,the grade of limb swelling,the occurrence time of the first skin wrinkle sign,CRP,WBC and the incidence of preoperative deep vein thrombosis,and the pain degree score,Kolaba comfort assessment scale score,adverse reactions and complications were the secondary observation indexes.SPSS26.0 was used for statistical analysis of relevant data.【Results】1 Through consulting authoritative literature and relevant books,and through the expert group meeting,this study finally formed three first-level indicators,nine second-level indicators,and 23 third-level indicators of the "Early swelling kinesio taping scheme for patients with closed fracture of the middle and lower third segment of the tibia".The expert authority coefficient(Cr)was 0.878(>0.8),indicating that the expert authority was high,The scheme is scientific and feasible.2 Evaluation of application effect of intramuscular effect patch(1)Comparison of baseline data between the two groups: there was no statistically significant difference between the control group and the test group in general demographic data,disease-related data and research-related observation indicators before intervention(P>0.05),which was comparable.(2)Analysis of main observation indicators:(1)Limb swelling value: There was no statistically significant difference between the two groups in limb swelling value on the first day after intervention(P>0.05);On the 3rd and 5th day after intervention,the limb swelling value of patients in the test group was lower than that in the control group(2.60±0.29 vs 2.87 ± 0.44,2.0 ± 0.20 vs 2.38 ± 0.39),and the difference was statistically significant(P<0.05).(2)Grade of limb swelling: On the fifth day after intervention,the proportion of patients with limb swelling above grade I in the test group was lower than that in the control group(25% vs 58.3%),and the difference was statistically significant(P<0.05).(3)The appearance time of the first skin wrinkle sign: the appearance time of the first skin wrinkle sign in the test group was shorter than that in the control group [3.5(3,4)vs 4(4,5)],and the difference was statistically significant(P<0.05).(4)Laboratory-related inflammatory indicators: There was no statistically significant difference in WBC between the control group and the test group on the fifth day after intervention(P>0.05);On the fifth day after intervention,the CRP of the test group was lower than that of the control group(10.12 ± 4.67 vs 12.84 ± 3.64),and the difference was statistically significant(P<0.05).(5)The incidence of preoperative DVT in the study group was significantly lower than that in the control group(0.0% vs 16.7%)(P<0.05).(3)Analysis of secondary outcome measures:(1)Pain degree: there was no statistically significant difference in NRS scores between the control group and the test group on the first day after intervention(P>0.05);The NRS score of the test group on the 3rd and 5th day after intervention was lower than that of the control group [3(3,3)vs 4(3,4),2(1.25,2)vs 3(2,3)],and the difference was statistically significant(P<0.05).(2)Kolaba comfort assessment scale score: the Kolaba comfort assessment scale score of the test group on the fifth day after intervention was higher than that of the control group(81.92±2.48 vs 74.63±3.31),and the difference was statistically significant(P<0.05).(3)Adverse reactions:There were no allergic reactions such as skin redness,rash,itching,burning pain and severe gastrointestinal reactions in both groups.(4)Complications: No patients in the two groups were complicated with osteofascial compartment syndrome.There was one case of tension blister in the control group,and there was no tension blister in the test group.There was no significant difference in the incidence of tension blister between the control group and the test group(P>0.05).【Conclusions】1 Through consulting the expert consensus and authoritative published books,and through the expert group meeting,this study has constructed the "Early swelling kinesio taping scheme for patients with closed fracture of the middle and lower third segment of the tibia",which is highly authoritative by experts,indicating that the content of this research scheme is feasible and scientific,and can provide reference and basis for the treatment of patients with early traumatic soft tissue swelling in orthopedics.2 After preliminary clinical verification,it was confirmed that the kinesio taping dressing scheme constructed in this study combined with conventional swelling reduction treatment had a faster and better effect on patients with orthopedic traumatic early swelling than conventional swelling reduction treatment alone.It can shorten the time of the appearance of skin wrinkles,reduce the concentration of CRP in patients,reduce the inflammatory reaction,and reduce the incidence of preoperative DVT in patients.KT treatment can also effectively relieve the pain feeling of patients and improve their comfort,Promote patients to perform functional exercises,which is conducive to the rehabilitation of limbs.This program shows that this scheme has certain clinical significance,and provides a scientific and operable basis for the application of kinesio taping technology in orthopedic traumatic early swelling,and has clinical promotion and application value. |
