| Objective:To analyze the onset,clinical features and prognostic factors of the sudden deafness patients with BPPV,and provide guidance and reference for clinical work.Materials and methods:Analyzed the 180 clinical information of patients treated in the department of Otorhinolaryngology Head and Neck Surgery of Affiliated Hospital of Binzhou Medical University,from September,2015 to September,2016 and diagnosed with sudden deafness..According to whether they were associated with BPPV,they were divided into sudden deafness with BPPV group(group A,27 cases)and without BPPV group(group B,153 cases).In addition,100 patients with idiopathic BPPV who came to our hospital at the same time were selected as group C.The sudden deafness patients with different hearing curves were treated with different therapeutic methods.All BPPV patients were treated with restoration.A comprehensive analysis of the general conditions,onset and clinical features,PTA,vestibular function,treatment and recurrence of the three groups were reviewed.Results:①The incidence of the sudden deafness patients with BPPV was about 15.0%,Female:male was11:10,the average age was(47.5± 15.4)years old,right side:left side was13:14.②The hearing loss mainly occurs earlier than vertigo(59.3%);The sudden deafness patients with BPPV mainly occurs in the posterior semicircular canal(55.26%);The mutiple semicircular-BPPV of the sudden deafness patients with BPPV(22.2%)were statistically significant(p<0.05)compared with the idiopathic BPPV(8.0%);The type of total deafness was the most hearing curve in the sudden deafness patients with BPPV(52.2%),and the difference was statistically significant(p<0.05)compared with the sudden deafness patients without BPPV(22.9%);44.4%of the sudden deafness patients with BPPV had cp(+),and the difference was statistically different(p<0.05)compared with the patients without BPPV(25.5%)·③In terms of the effectiveness of hearing recovery,the sudden deafness with BPPV group(37.0%)was lower than the sudden deafness without BPPV group(60.1%),and the difference was statistically significant(p<0.05).The type of total deafness was the worst.④The sudden deafness patients with BPPV and the idiopathic BPPV patients were cured after 1-5 times treatment.The sudden deafness with BPPV patients recovered after 3.28±1.81 times treatment,and the difference was statistically significant(P<0.05)compared with the idiopathic BPPV patients(1.45±1.62 times).The sudden deafness with BPPV patients was subdivided into CP(+)and CP(-)groups for statistical analysis.The patients in CP(+)group recovered after 3.58±0.99 times treatment,and the difference was statistically significant(P<0.05)compared with the patients in CP(-)group(1.45± 1.62 times).However,the CP value is not proportional to the number of treatment.⑤The recurrence rate of the sudden deafness patients with BPPV(11.1%)was higher than that of the idiopathic BPPV(4.0%).There was no significant difference between the two groups(p<0.05).Conclusions:①In the sudden deafness patients with BPPV,The onset of vertigo was mainly later than hearing loss;The P-BPPV is the most common,the Probability of M-BPPV was higher than the patients with idiopathic BPPV;the Abnormal rate of CP was higher than the sudden deafness patients without BPPV.②The hearing prognosis of the sudden deafness patients with BPPV is worse than that of the ISSHL patients without BPPV.In patients with sudden onset of BPPV,the prognosis of total hearing loss is the worst.The type of total deafness was the worst.③Compared with the idiopathic BPPV patients,when the sudden deafness patients with BPPV is cured,the times of restoration is more,but there is no difference between the recovery rate and the recurrence rate.When CP abnormality occurs in the sudden deafness patients with BPPV,the times of restoration needs to be increased,and the times of restoration is not proportional to the CP value.④The possible causes of sudden onset of BPPV include inner ear microcirculatory disorder and inner ear virus infection,but it is not clear. |
