Objective To explore the expression of tumor stem cell markers Lgr5 and CD24 in colorectal adenomas,and analyze the relationship between them and TCM syndrome types,so as to provide a new idea for clinical prevention,early diagnosis and early treatment of colorectal adenomas.Methods From September 2018 to October 2019,200 patients who make electronic colonoscopy and pathological diagnosis of colorectal polyps and met the inclusion criteria and exclusion criteria were selected from the endoscopy center of Hangzhou hospital of traditional Chinese medicine.According to the classification criteria,there were five types:dampness heat retention type,spleen kidney yang deficiency type,liver stagnation type,spleen deficiency with stasis type.At the same time,according to the pathological classification standard of colorectal polyps,they were divided into non-neoplastic polyp group and colorectal adenoma group.In addition,30 normal patients were selected as the control group.The expression of CD24 and Lgr5 were observed by immunohistochemical staining and the results were collected.The expression of the two factors and the relationship between the two factors and TCM syndrome types were summarized by statistical analysis.Results 1.The age and gender of normal mucosa group,non-neoplastic polyp group and adenoma group were compared,and the expression of TCM syndrome type in non-neoplastic polyp group and adenoma group were compared,P>0.05,the difference was not statistically significant and comparable.2.The expression of Lgr5 was related to pathological classification,and the positive rate increased gradually in normal mucosa,non-neoplastic polyp group and adenoma group(P<0.05).The difference between adenoma group and normal mucosa group and non-neoplastic polyp group was statistically significant(P<0.05);the expression of Lgr5 in adenoma group was not related to age and gender of patients(P>0.05);the expression of Lgr5 in adenoma group was related to TCM syndrome type,and the expression in the syndrome of dampness heat retention in intestine was significantly higher than that in other groups.The difference was statistically significant(P<0.05).3.The expression of CD24 was related to pathological classification,and the positive rate increased gradually in normal mucosa,non-neoplastic polyp group and adenoma group(P<0.05).The difference between adenoma group and normal mucosa group and non-neoplastic polyp group was statistically significant(P<0.05);the expression of CD24 in adenoma group was not related to age and gender of patients(P>0.05);the expression of CD24 in adenoma group was not related to TCM syndrome type.Conclusion 1.The expression of Lgr5 and CD24 increased gradually in normal mucosa,non-neoplastic polyps and adenomas.It is suggested that Lgr5 and CD24 may be early predictors of colorectal precancerous lesions.2.The expression of Lgr5 in adenoma may be related to the syndrome type of traditional Chinese medicine.The positive rate of Lgr5 is higher in the syndrome of dampness heat retention in intestine and the syndrome of spleen deficiency and blood stasis,but there is no guidance for the differentiation between the two syndromes.3.There was no correlation between the expression of CD24 in adenoma and TCM syndromes.4.The expression of Lgr5 and CD24 was not related to the gender and age of patients.
