Objective:To investigate the effect of oral L-carnitine on improving the process of autologous vein graft restenosis after coronary artery bypass grafting in rats,and to explore the inhibitory effect of L-carnitine on postoperative autologous vein bridge restenosis,and provide new methods of improving and inhibiting postoperative autologous vein bridge restenosis.Methods:1.Establishment of a rat model of external jugular vein abdominal aorta.Male adult Wistar rats(weight 250g-300g)were randomly divided into 4 groups:normal control group(n=21,no transplantation,only vein separation);vein graft group(n=21,vein transplantation only);soybean peptide control group(n=21,vein transplantation+oral soy peptide nutrient solution);oral soybean peptide and L-carnitine mixed group(n=21,vein transplantation+oral L-carnitine).2.Feeding and administration.the oral soybean peptide and L-carnitine mixed group was injected with 2ml mixture of L-carnitine and soybean peptide.The rats in the soybean peptide control group were fed with soybean peptide nutrient solution 2ml/only.The time of intragastric administration was about 5 p.m.each day,each group was fed with normal diet,and each group was given 20g food daily,drinking water freely.The operation lasted for 1 weeks.Follow up the original group and continue feeding for one week.3.Measurement and evaluation of the degree of restenosis of autogenous vein bridge.the internal diameter of the vein bridge was measured by ultrasound,and the restenosis status was assessed.Pathological examination of vein bridge restenosis was performed by Masson staining,HE staining and immunohistochemical staining.4.Statistics and analysis.through SPASS,Ameria and other software analysis of data,analysis of L-carnitine and coronary artery bypass graft autologous vein bridge restenosis process correlationResults:1.A rat model of autologous external jugular vein abdominal aorta was successfully established2.The color Doppler ultrasound examination showed that the oral soybean peptide and L-carnitine mixed group could reduce the vein graft stenosis and improve the hemodynamics of transplanted vessels in rats3.HE evaluation of intimal hyperplasia staining confirmed that the oral soybean peptide and L-carnitine mixed group(56.83±9.99?m)was better than the vein graft group(127.3±16.64?m),the soybean peptide control group(141.7±13.12?m),and the intimal thickening obviously decreased,but not same as the control group(23.24±6.26?m).The results of Masson staining showed that the extracellular matrix of the oral soybean peptide and L-camitine mixed group(60.33±13.88%),the vein graft group(67.83±10.05%),and the soybean peptide control group(68.50±10.56%)was basically no difference.and CD68 immunohistochemistry showed that the level of infiltration of inflammatory cells in oral soybean peptide and L-carnitine mixed group(14.67±.04/view)is lower than the vein graft group(28±4/view),the control group(12.56±3/view),and the soybean peptide control group(26.17±2.7/view)Conclusion:1.Oral carnitine can reduce the invasion of inflammatory cells,improve the intimal hyperplasia of transplanted vessels,improve the blood flow of vein graft,and delay the process of restenosis.2.The transportation of multiple inflammatory factors and metabolites such as TNF-a?,interleukin and the inhibitory effect of NF-?B could be the possible mechanism.The oral administration of L-carnitine provides a new perspective for the prevention and treatment of restenosis.
