Objective:To compare the differences in AR-V7,NLR,ISUP grouping,PSA nadir,PSA velocity,age,and castration mode in castration-resistant prostate cancer(CRPC)and castration-sensitive prostate cancer(CSPC),and to explore the associated factors to CRPC.Method:A retrospective analysis was performed to collect inpatient medical records and outpatient follow-up data of patients diagnosed with prostate cancer by the department of pathology of Affiliated Hospital of North Sichuan Medical College from january 2016 to december 2017.according to the inclusion and exclusion criteria,the cases were divided into castration resistant prostate cancer group(observation group)and castration sensitive prostate cancer group(control group)according to follow-up data.The expression of AR-V7 in observation group and control group was detected by immunohistochemistry,The differences of NLR,ISUP group data,PSA nadir and the PSA velocitywere compared,compare the indexes such as AR-V7 expression level of the two groups of patients,multivariable logistic regression was assessed for factors with statistical probability of P<0.05,to investigate the correlation between the above indicators and CSPC progress to CRPC.Result:1.1 Immunohistochemical results showed that AR-V7 positive staining was mainly located in the nucleus or a small amount of cytoplasm,with brownish-yellow granules.Among the 21 patients in the observation group,there were 18(85.7%)patients with AR-V7 positive immunohistochemical stains.Among the 61 cases in the control group,there were 26(42.6%)patients with AR-V7 positive immunohistochemical stains.The difference was statistically significant(85.7%vs42.6%P<0.05).1.2 Among the 21 patients in the observation group,there were 8(38.1%)patients with NLR less than 3.3,Among the 61 patients in the control group,there were 39(63.9%)patients with NLR less than 3.3,The difference was statistically significant(38.1%vs63.9%P<0.05).1.3 Among the 21 cases in the observation group,there were 2(9.52%)patients with ISUP group ?3,5(23.81%)patients with ISUP group=4,14(66.67%)patients with ISUP group=5.Among the 61 patients in the control group,there were 25(40.98%)patients with ISUP group ?3,10(16.39%)patients with ISUP group=4,26(42.62%)patients with ISUP group=5.The difference was statistically significant(?2=7.017,P<0.05).1.4 Among the 21 cases in the observation group,there were 3(14.3%)patients with PSA nadir less than 0.2 ug/L,among the 61 patients in the control group,29(47.5%)patients with PSA nadir less than 0.2 ug/L.The difference was statistically significant(14.3%vs47.5%P<0.05).1.5 In addition,the difference between the observation group and the control group was statistically significant in PSA velocity(57.49(11.60?112.30)vs16.94(5.70?58.34)P<0.05).1.6 The difference between the observation group and the control group in age(71.62±6.25 vs 74.03±6.75 P>0.05)and castration mode(52.38%vs39.34%P>0.05)was not statistically significant.2 Logistic regression analysis showed that the expression of AR-V7 protein(OR=5.149,95%CI:1.098-24.136,P<0.05)was an independent risk factor for the occurrence of CRPC.While NLR,ISUPgrade,PSA nadir,PSA velocity were not related to the occurrence of CRPC.(P>0.05).Conclusion:In the univariate analysis:expression of AR-V7 protein,NLR,ISUPgrade,PSA nadir,PSA velocity were associated with the occurrence of CRPC;but there was no statistically significant on age,castration mode.In multifactorial analysis:the expression of AR-V7 protein is an independent risk factor for the occurrence of CRPC.
