Effect Of Autologous Platelet-rich Plasma(PRP)on Chronic Wound Healing In Different Age Groups

Objective:To observe and evaluate the effect of autologous whole blood PRP on chronic wound healing.Methods:60 patients with chronic wound were selected from June 2018 to June 2019.Patients who met the inclusion criteria were divided into three groups according to age:youth group(25-44 years),middle-aged group(45-64 years),elderly group(over 65 years),20 patients in each group.The general condition of the patients was evaluated after admission,the VAS pain score was carried out,the internal diseases of the patients were actively treated,after the condition was stable,the wound was debridement,the secretion was cultured by bacteria,according to the size of the wound,a certain amount of the patient's own whole blood was extracted,and the PRP gel was made and applied to the wound.The wound condition of the patient was treated with dressing change.observe and evaluate the wound healing rate,bacterial yin conversion rate and VAS pain score of the three groups of patients at different time points of the wound.Results:1.After treatment,the wounds of the three groups tended to heal,and healed to varying degrees on days 7,10,and 14 after treatment.The difference in wound healing rate was statistically significant.(P<0.05).There was no significant difference in the wound healing rate between the youth group and the middle-aged group on the seventh and tenth days(P>0.05),while in the youth group on the fourteenth day(73.63±16.01%),the wound healing rate was higher than that in the middle-aged group(62.35±15.10%),and the difference was statistically significant(P>0.05).Wound in middle age group on day 7 the healing rate(20.29± 11.37%)was higher than that of the elderly group(12.56±5.91%),and the difference was statistically significant(P<0.05),while the wound healing rate of the two groups did not show a significant difference on the tenth and fourteenth days(P<0.05).The wound healing rate(23.21 ±12.17%)on the seventh day,the wound healing rate(43.06± 14.06%)on the tenth day and the wound healing rate(73.63±16.01%)on the fourteenth day were higher in the youth group than in the old group(12.56±5.9 The wound healing rate on day 10(32.15±11.65%)and day 14(54.22± 16.54%)were statistically significant(P<0.05).2.There was significant difference in the overall negative rate of bacteria in the three groups after the seventh and fourteenth days of treatment(P<0.05).There were 8 positive bacterial cultures before treatment in the young group,only 4 positive bacterial cultures after seven days of treatment,and the negative rate of bacteria was 50%;only 1 positive bacterial culture after 14 days of treatment,the negative rate of bacteria was 87.5%.In the middle-aged group,13 patients had positive bacterial culture before treatment,seven patients had positive bacterial culture after seven days of treatment,and the negative rate of bacteria was 46.2%;after 14 days of treatment,4 patients had positive bacterial culture,and the negative rate of bacteria was 69.2%.14 bacteria before treatment in elderly group culture positive,after seven days of treatment,12 cases of bacterial culture positive,bacterial negative conversion rate of 14.3%;after 14 days of treatment,10 cases of positive bacterial culture,bacterial negative conversion rate of 28.6%.Both the youth group and the middle-aged group showed no significant statistical difference on the seventh and fourteenth days after treatment(P>O.05).On the seventh day,there was no statistical difference between the middle-aged group and the elderly group(P>0.05),and on the fourteenth day,the middle-aged group had a higher bacterial negative rate than the elderly group.The rate of bacterial negative conversion was statistically significant(P<0.05).The rate of bacterial negative conversion in the young group was higher than that in the elderly group on the seventh and fourteenth days,and the difference was statistically significant(P<0.05).3.After seven days of treatment,the pain of the three groups of patients was significantly relieved compared with that before surgery,and the VAS scores were different,with statistical significance(P<0.05).There was no statistically significant difference in the VAS score between the young group and the middle-aged group after the seventh day of treatment(P>0.05).After the seventh day of treatment,the VAS score of the middle-aged group(2.90± 1.17)was lower than that of the elderly group(3.80±1.24),and the difference was statistically significant(P<0.05).After the seventh day of treatment,the.VAS score of the youth group(2.35±1.04)was significantly lower than that of the elderly group(3.80±1.24),and the difference was statistically significant(P<0.05).Conclusion:1.As the patient's age increases,the autologous platelet-rich plasma(PRP)has a weakening effect on the promotion of chronic wound healing.2.Autologous platelet-rich plasma(PRP)in young and middle-aged patients significantly promoted the healing of chronic wounds,and autologous platelet-rich plasma(PRP)in elderly patients over 65 years of age significantly decreased the promoting effect on chronic wound healing.
