Objective:To figure out the meaningful parameters from the analysis of the clinical data of 827 cases of prostate biopsy and compare the positive rate and complication rate in different pathways and methods of biopsy to help select the best way for prostate biopsy.Methods:Clinical data of prostate biopsy of the First Affiliated Hospital of Soochow University was collected from June 2015 to September 2018.827 cases of prostate biopsy were selected.All the patients were divided into Group PCa(Prostate Cancer)and Group BPH(Benign Prostatic Hypertrophy)according to the pathological results.Age,BMI,hypertension,diabetes,tPSA,f/t PSA,PSAD,transversal diameter,anteroposterior diameter,superoinferior diameter,PV(Prostate Volume),DRE(Digital Rectal Exam)and MRI diagnosis were analyzed and compared between two groups.The difference of positive rate and complication rate of biopsy between transrectal puncture and transperineal puncture was also compared.Also,the difference of positive rate and complication rate of biopsy between MRI targeted biopsy and systematic biopsy was collected and compared.For patients of MRI targeted biopsy,the number of positive tissues of systematic punctures and MRI guided punctures were collected and comparedResults:Age of the patients ranged from 30 to 88,the average age was(67.91±8.39).A total of 391 cases were pathological confirmed PCa and the rest were diagnosed with BPH.The overall positive rate was 47.28%.There were significant differences between Group PCa and Group BPH in age,hypertension patients,tPSA.f/t PSA,PSAD,transversal diameter,anteroposterior diameter,superoinferior diameter,PV,prostatic rigidity,patients of prostatic nodule,patients of MRI positive diagnosis(P<0.05).In patients taken MRI-targeted biopsy,140 patients out of 264 patients using transperineal biopsy were dignosed PCa while 104 patients out of 186 patients using transrectal biopsy were dignosed PCa(P>0.05).Hematuria was more common in transrectal biopsy compared with transperineal biopsy(P>0.05).And in patients taken systematic biopsy,33 patients out of 72 patients using transperineal biopsy were dignosed PCa while 114 patients out of 305 patients using transrectal biopsy were dignosed PCa(P>0.05).Hematuria was more common in transrectal biopsy compared with transperineal biopsy(P<0.05).In patients taken transperineal biopsy,140 patients out of 264 patients using MRI-targeted biopsy were diagnosed PCa as well as 33 patients out of 72 patients using systematic biopsy were diagnosed PCa(P>0.05).In patients taken transrectal biopsy,104 patients out of 186 patients using MRI-targeted biopsy were diagnosed PCa as well as 114 patients out of 305 patients using systematic biopsy were diagnosed PCa(P<0.05).There were 1262 positive tissues out of the 5355 punctures of the systematic biopsy and 394 positive tissues out of the 1149 punctures of MRI targeted biopsy.MRI targeted biopsy had a higher positive rate compared to systematic biopsy(30.83%vs.52.19%,P<0.05).Binary Logistic Analysis revealed that age,PSAD,prostatic rigidity and MRI positive diagnosis were all independent meaningful factors associated with PCa.Conclusion:The meaningful paraments related to positive prostate biopsy included age,PSAD,prostatic rigidity and MRI diagnosis.There was no significant difference in positive rate between transperineal biopsy and transrectal biopsy while hematuria was more common in transrectal biopsy.The positive rate of MRI-targeted biopsy was superior to systematic biopsy.
