| Objective:To observe the relationship between serum asymmetric dimethylarginine(ADMA)level and blood pressure variability(BPV)in patients with essential hypertension complicating acute cerebral infarction.Methods:1.A total of 125 patients with essential hypertension from Lianyungang Second People’s hospital were enrolled and divided into essential hypertension with acute cerebral infarction group(n=80,47 male,33 female,aged 40 to 80 years old,mean age 66.01 ± 10.74 years)and simple essential hypertension group(n=45,20 male,25 female,aged 40 to 80 years old,mean age 66.1 6± 10.43 years)according to whether they complicated with acute cerebral infarction.The two groups were selected from hospitalized patients at the same time,and the the ratio of gender and age were matched.2.Detection indicators:All patients were monitored for 24-hour ambulatory blood pressure and BPV indexes such as standard deviation(SD)and variation coefficient(CV)were recorded.The general clinical data of the patients,such as gender,age,hypertension,diabetes,coronary heart disease,smoking history were recorded and their serum ADMA levels were measured by enzyme-linked immunosorbent assay(ELISA).At the same time,the international standardized ratio(INR),High-sensitivity C-reactive protein(hs-CRP),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),creatinine(Cre)and Fasting blood glucose(FBG)of all subjects were detected.3.To analyze the data,SPSS 25.0 statistical software was used to compare the levels of serum ADMA and BPV between the two group and to explore the relationship between serum ADMA level and BPV index such as SD and CV in patients with essential hypertension complicating acute cerebral infarctionResults:1.Compared with the simple essential hypertension group,there was no statistically significant difference in gender,age,smoking history,INR,TC,TG,Cre and FBG between the two groups(P>0.05);while the levels of hs-CRP,HDL-C,and LDL-C were statistically significant between the two groups(P<0.05).2.The level of serum ADMA in essential hypertension with acute cerebral infarction group(1.49 ± 0.39 μmol/L)was higher than that in simple essential hypertension group(1.12±027 μmol/L),and the difference was statistically significant(P<0.05).3.Standard deviation(SD)and coefficient of variation(CV)were used as indicators to evaluate BPV.BPV indicators such as dSSD(15.67±3.50)、nSSD(13.12±3.44)、24hDSD(10.87±2.41)、nDSD(9.47±2.31)、24hSCV(0.13±0.34)、dSCV(0.12±0.72)、nSCV(0.11±0.04)、24hDCV(0.14±0.03)、dDCV(0.13±0.04)and nDCV(0.13±0.04)in essential hypertension with acute cerebral infarction group were significantly higher than those in the simple essential hypertension group(13.75±2.84,11.43±3.85,9.87±2.45,7.76±2.48,0.10±0.22,0.09±0.22,0.10±0.02,0.12±0.03,0.09±0.02,0.11±0.03,P<0.05).4.Pearson linear correlation analysis showed that 24hSSD、dSSD、dDSD、24hSCV、dSCV、nSCV、24hDCV were positively correlated with the level of serum ADMA(r=0.28,0.23,0.28,0.53,0.37,0.39,0.26,P<0.05)of patients with essential hypertension complicating acute cerebral infarction.Moreover,Multiple linear regression analysis showed that the level of serum ADMA was independent influencing factor for 24hSSD、dDSD、24hSCV and nSCV of patients with essential hypertension complicating acute cerebral infarction.Regression coefficient B=3.70,2.48,0.06,0.03,P<0.05,adjusted R-squared=0.08,0.06,0.32,0.23Conclusions:1.The level of serum ADMA of patients with essential hypertension complicating acute cerebral infarction was higher than that of patients with simple essential hypertension.2.BPV indicators such as dSSD、nSSD、24hDSD、nDSD、24hSCV、dSCV、nSCV、24hDCV、dDCV、nDCV of patients with essential hypertension complicating acute cerebral infarction were higher than those of the simple essential hypertension group 3.24hSSD、dSSD、dDSD、24hSCV、dSCV、nSCV and 24hDCV were positively correlated with the level of serum ADMA of patients with essential hypertension complicating acute cerebral infarction.In this study,we found that the level of serum ADMA was an independent influencing factor for 24hSSD、dDSD、24hSCV and nSCV in patients with essential hypertension complicating acute cerebral infarction.However,due to the small sample size,further large-scale clinical studies were needed to obtain the conclusion of independent influencing factors. |
