| Objective:Blood lipids and thyroid function have important effects on the occurrence and development of pre-eclampsia.This study retrospectively analyzes the indicators of blood lipids and thyroid function in pre-eclampsia patients and normal pregnant women,and analyzes various indicators and pre-eclampsia.The relationship,and explore the preliminary predictive value of the indicators for individual and combined detection of pre-eclampsia,to provide a reasonable and effective way of managing pregnancy and prevention of pre-eclampsia.Methods:One hundred patients with pre-eclampsia who were hospitalized at the Affiliated Hospital of Binzhou Medical College from January 01,2017 to December 30,2017 were selected as PE group.Among them,50 patients with mild pre-eclampsia were MPE group,severely in the pre-eclampsia,50 patients were in the SPE group,and 100 normal pregnant women who gave birth at the same time were selected as the control group.The medical records of the subjects and the indicators of blood lipids and thyroid function were recorded.The differences of blood lipids and thyroid function indexes between the groups were analyzed and compared.Logistics regression model was used to evaluate the correlation between each index and pre-eclampsia.The predictive value of each index for pre-eclampsia was evaluated by the Receiver Operating Characteristic Curve(ROC)curve.Result:1.The levels of TG,TC and LDL in the SPE group and the MPE group were significantly higher than those in the control group,and the difference was statistically significant(Pa,Pb=0.000),while the SPE group and the MPE group were TG,LDL,TC.There was no significant difference in the odds ratio(P=0.180,0.336,0.475),but the levels of TG,LDL,and TC increased with the severity of the disease;there was no significant difference in HDL levels between the SPE group,the MPE group,and the control group(Pa=0.593,Pb=0.169,Pc=0.295).2.The levels of TSH in the SPE group and the MPE group were signif icantly higher than those in the control group(Pa=0.000,Pb=0.022),but the re was no significant difference between the SPE group and the MPE group.Pc=0.180),but it can be seen that the level of TSH is gradually increasing with the severity of the disease;the FT4 level in the SPE group and the MPE group is lower than that in the control group,and the difference is statistically significant(Pa=0.014,pb=0.020),and the difference between the SPE group and the MPE group was not statistically significant(Pc=0.267),while the FT4 level gradually decreased with the severity of the disease;th e SPE group,the MPE group,and the control group FT3 There was no sta tistically significant difference in horizontal contrast(Pa=0.096;Pb=0.428,Pc=0.475).3.Analysis by logistic regression model,TG(OR=3.285,P=0.000),LDL(OR=1.538,P=0.010),TSH(OR=2.011,P=0.000),FT4(OR=0.721,P=0.003)Meaningful.Elevated levels of TG,LDL,TSH and decreased FT4 levels are risk factors for PE,and TG has the greatest impact on the incidence of PE.4.The results of ROC curve showed that the areas under the ROC curve of pre-eclampsia were 0.786,0.681,0.781,and 0.652,respectively,and the best cut-off values were 2.58 mmol/L and 3.84 mmol/L,respectively,2.64mIU/L,11.25pmol/L.When the optimal cutoff value is obtained,the LDL,TG,TSH and FT4 sensitivity are 65.0%,87.8%,79.6%,75.5%,respectively,and the specificity is 87.0%.41.1%,65.1%,49.6%;The three indicators of TG,LDL and TSH combined predict the area under the ROC curve of pre-eclampsia is 0.831,the sensitivity is 82.7%,and the specificity is 76.7%.The four indicators of TG,LDL,TSH and FT4 combined to predict the area under the ROC curve of pre-eclampsia was 0.907,the sensitivity was 89.8%,and the specificity was 78.3%.Conclusion1.The levels of TG,TC and LDL in patients with pre-eclampsia were significantly higher than those in normal pregnancy,and there was no significant change in HDL level.The elevated levels of TG and LDL are risk factors for pre-eclampsia,and have certain predictive value for pre-eclampsia.2.The pre-eclampsia patients were significantly higher than the normal pregnancy TSH,the FT4 level was significantly decreased,and the FT3 was not significantly changed.The increase of TSH level and the decrease of FT4 level are risk factors for pre-eclampsia,and have certain predictive value for pre-eclampsia.3.The combined predictive value of combined TG,LDL,TSH and FT4 for pre-eclampsia was significantly higher than that of separate detection.4.For pregnant women with high risk factors of pre-eclampsia,attention should be paid to the screening of blood lipids and thyroid function,which can make a preliminary prediction of the occurrence of pre-eclampsia,and timely intervention for those with abnormalities to reduce the risk of pre-eclampsia. |
