| Objective:To explore the application of enhanced recovery after surgery(ERAS)concept in the early operation of femoral neck fracture in the elderly and to compare multiple indexes of patients with elective operation,so as to provide theoretical support and clinical basis for the operation timing of the elderly patients with femoral neck fracture. Methods:Methods Retrospective analysis was used to collect the elderly patients with femoral neck fracture who underwent surgery in the Second Affiliated Hospital of Kunming Medical University from May 2017 to June 2019.90 subjects were selected according to the inclusion criteria and exclusion criteria. After June 2018,39 elderly patients with femoral neck fracture who applied era concept were in rapid rehabilitation surgery group(ERAS Group),and 51 patients who did not use era concept before June 2018 were in traditional treatment mode group(conventional group). The general data(sex, age, injury side),operation time, intraoperative hemorrhage, postoperative complications, hospital stay,Harris score of hip joint function half a year after operation and mortality within one year were compared between the two groups to evaluate the treatment effect.Through the statistical data of the two groups of cases,to explore the effect of enhanced recovery after surgery in the perioperative period of the elderly femoral neck fracture. Results:There was no significant difference between the two groups in age, gender and injury side(P>0.05).In ERAS group, the operation time was(117.69±16.18)min,intraoperative bleeding volume was(272.56 ± 122.21)ml,postoperative complications occurred in 4 cases, average hospitalization time was(9.51 ±2.13)days.hip function evaluation score was(86.23±3.76),and the death toll was 3 in one year.In the routine group, the operation time was(115.79±29.13)min,the intraoperative bleeding volume was(337.25 ±132.61)ml,the postoperative complications occurred in 18 cases, the average hospitalization time was(12.53 ±2.51)days,the hip joint function evaluation score was(84.24±5.15),and the death toll in one year after operation was 5.There was no significant difference in the operation time between ERAS group and routine group(P>0.05);there was no significant difference in the blood loss between ERAS group and routine operation group(P>0.05);there was significant difference in the postoperative complications between ERAS group and routine group(P<0.05);there was significant difference in the average hospital stay time between ERAS group and routine group(P<0.05);hip joint function evaluation of ERAS group after operation. The difference between the two groups was statistically significant(P<0.05).There was no significant difference in one-year mortality rate(P>0.05)Conclusion:The application of enhanced recovery after surgery(ERAS)in the perioperative period of elderly femoral neck fracture is conducive to reduce the incidence of postoperative complications, shorten the average hospital stay, and promote the recovery of hip joint function. |
