Objective(s):To retrospectively analyze the long-term prognosis of benign childhood epilepsy with centrotemporal spikes,and then to explore the high risk factors affecting poor seizure control in children with BECT.Methods:From January 2016 to March 2019,93 children with BECT were enrolled in the Department of Pediatrics,First Affiliated Hospital of Kunming Medical University.All enrolled children were treated with antiepileptic drugsand returned to the hospital regularly.They were followed up regularly for more than 1 year.The follow-up included whether there were clinical seizures,medication use,EEG re-examination results,etc.Complete follow-up data were recorded,and their clinical characteristics,disease evolution and prognosis were analyzed.According to whether there were clinical episodes after 1 year of medication,they were divided into well-controlled group(48 cases)and poor-controlled group(45 cases).Firstly,the differences in gender,age at onset,history of febrile convulsion,family history of epilepsy,seizure form,seizure time,EEG discharge location,discharge index and treatment drugs between the two groups were compared by t test or 2 test according to the data category,and then the factors with statistical significance were analyzed by logistic regression to further screen out the factors affecting the poor seizure control of children with BECT.Results:Among 93 children with BECT,48(51.61%)were in the well-controlled group and 45(48.39%)were in the poor-controlled group.Preliminary statistical analysis showed that there were significant differences between the two groups in onset age,number of seizures before treatment,EEG discharge index,abnormal discharge during waking period,migration of epileptic focus,and initial drug treatment.Further logistic regression analysis showed that onset age(P=0.048,OR=0.833,95%CI:0.694-0.998),EEG discharge index(P=0.042,OR=3.726,9.5%CI:1.050-13.218),abnormal discharge during awake period(P=0.011,OR=3.730,95%CI:1.359-10.237)were the related factors affecting the poor prognosis of children with BECT,while gender,frequency of seizures(number of seizures before treatment),seizure time,seizure form,duration,EEG discharge side,discharge concentration site,discharge synchrony,epileptic focus wandering,history of febrile convulsion,family history of epilepsy and other factors were not associated with poor seizure control in children with BECT(all P>0.05).Conclusion(s):The study showed that most BECT have a good prognosis,and the high risk factors affecting the poor seizure control include younger age at onset(?5 years),higher electroencephalogram discharge index(?50%),and presence of Rolandic area discharge during awake period.
