Objective:The levels of IL-22,leukocytes and neutrophils in peripheral blood of COPD patients with different airflow restriction in acute exacerbation and remission were measured to study the significance of the above indexes in the occurrence and development of COPD.Methods:From April 2018 to December 2019,COPD patients in the second ward of respiratory and Critical Medicine Department of Kunming Medical University Second Affiliated Hospital in the acute exacerbation stage were examined with the German KANGXUN PowerCube BODR pulmonary function instrument,and chest CT was selected There were 78 cases of COPD in acute exacerbation stage without obvious exudation and PCT<0.1ng/ml,including 19 cases of mild(FEV1%pred? 80),20 cases of moderate(80%>FEV1%pred? 50),20 cases of severe(50%>FEV1%pred?30),19 cases of extremely severe(FEV1%pred<30).On the day of admission and in the clinical remission period after treatment,the level of leukocytes and neutrophils in the peripheral blood of the subjects were detected by the automatic hematology analyzer;peripheral blood IL-22 levels were detected by ELISA on the second day after admission and in the clinical remission stage after treatment.Results:1.There was no significant difference in sex(P=0.959),age(P=0.992),smoking index(P=0.587)and admission PCT(P=0.592)among the four groups.2.There was a significant difference in BMI between the two groups(P=0.000)in the acute exacerbation stage of COPD,that is,BMI decreased significantly with the increase of the degree of airflow restriction in the acute exacerbation stage of COPD.3.There were significant differences in IRV and IC indexes of lung function between each group in the acute exacerbation stage of COPD(P=0.000).There was no significant difference in RV/TLC indexes of lung function(P=0.406).That is to say,IRV and IC decreased significantly with the increase of airflow restriction degree of COPD in the acute exacerbation stage.RV/TLC did not increase with the increase of airflow restriction degree of COPD in the acute exacerbation stage Increase or decrease significantly.4.Between the four groups of subjects with acute aggravating period of peripheral blood levels of IL-22 have significant difference(P<0.025),between the four groups remission peripheral blood levels of IL-22 have significant difference(P<0.025),and each group of acute aggravating period of peripheral blood of IL-22 and remission compared to peripheral blood IL-22 had significant difference(P<0.01),namely with the increase of the degree of acute aggravating period of COPD airflow limited,a significant rise in peripheral blood levels of IL-22,with the increase of remission COPD airflow limited extent,significantly increased the level of IL-22 of peripheral blood;The level of IL-22 in peripheral blood during COPD acute exacerbation was significantly higher than that in COPD remission(P<0.01).5.Between the four groups of subjects with acute aggravating period of peripheral blood leukocyte counts were no significant difference(P>0.05),four groups of remission between peripheral blood leucocyte count were no significant difference(P>0.05),however,each period of acute exacerbation and remission compared to peripheral blood WBC count had significant difference(P<0.01);The white blood cell count in the peripheral blood during COPD acute exacerbation was significantly higher than that in the peripheral blood during COPD remission(P<0.01).6.Between the four groups of subjects with acute aggravating period of peripheral blood neutrophil count had no significant difference(P>0.05),between the four groups remission of peripheral blood neutrophil count had no significant difference(P>0.05),however,each period of acute exacerbation and remission compared to peripheral blood neutrophil count had significant difference(P<0.01);Neutrophil count in peripheral blood during COPD acute exacerbation was significantly higher than that in peripheral blood during COPD remission(P<0.01).7.There was a negative correlation between the peripheral blood IL-22 level and FEV1,FEV1%pred,IRV,IC in the acute exacerbation stage of COPD(r=-0.658,r=-0.668,r=-0.0.549,r=-0.580),while there was no correlation between the peripheral blood IL-22 level and RV/TLC in the acute exacerbation stage of COPD.There was a positive correlation between FEV1%pred and BMI,FEV1,IRV and IC in patients with acute exacerbation of COPD(r=0.656,r=0.866,r=0.794,r=0.779),while there was no correlation between FEV1%pred and RV/TLC in patients with acute exacerbation of COPD.Conclusion:1.With the increase of the degree of airflow restriction in COPD,the level of IL-22 in peripheral blood increased significantly;2.IL-22,leukocytes and neutrophils are involved in the acute exacerbation of COPD;3.IRV,IC are positively correlated with FEV1%pred;4.With the increase of airflow restriction degree in COPD,the body mass index of patients decreased significantly.
