Background and objective Among Non-small cell lung cancer(NSCLC)patients,the elderly accounts for about the majority.The high mortality is caused by distant metastasis.The aim of the study was to confirmed the relationship between tumor size and metastatic site in stage IV NSCLC elderly patients.Methods A total of 33418 elderly patients with NSCLC diagnosed between2010 and 2015 were enrolled from Surveillance,Epidemiology,and End Result(SEER)databases.Basic patient data collected included race(white vs black vs others),sex(male vs female),marital status(married vs others),histology(adenocarcinoma vs squamous vs others),tumor size,lymph node stage and distant metastasis.We also collected 348 clinical elderly cases with T4 NSCLC.Multivariate logistic regression analysis was used to demonstrate the association between tumor size and distant metastasis site.Results For brain metastasis,the results indicated that tumor size is an independent risk factor(T2 VS T1,OR=1.159;T3 VS T2: OR=1.246;T4 VS T3: OR=1.266,P< 0.0001).For lung metastasis,the results revealed that patients with T2 had no significant difference compared to T1,while the statistical significance was found for patients with T3 and T4 compared to T1(T3 VS T1,OR=1.094 P=0.005;T4 VS T1,OR=1.323,P<0.0001).For liver metastasis,patients with T3 has slightly higher risk of develop.Metastasis compared to T1(T3 VS T1,OR=1.094,P=0.015).However,for bone metastasis,patients with T4 were less likely to develop Metastasis compared to T1(OR=0.855,P<0.0001)Conclusion There was statistical significance between tumor size and metastatic site in elderly patients with stage IV NSCLC.The larger the tumor,the higher the risk of brain metastasis.For lung metastasis,patients with T3 and T4 were more prone to develop metastasis than T1.For liver metastasis,patients with T3 were more likely to develop metastasis than T1.For bone metastasis,patients with T4 were less likely to develop metastasis than T1.
