Background: Since Choukroun and others prepared a new generation of platelet concentrate Platelet Rich Fibrin(PRF),it has caused a strong response.Because of its good repair function,it has been widely used in bone regeneration,cartilage repair,and various oral Treatment of maxillofacial surgical diseases and other surgical fields.For wounds formed by trauma,tumors,burns,scars and other factors,skin grafting is often used as a clinical repair method.Evidence-based medical research has shown that medium-thickness skin grafts have the advantages of friction resistance and less shrinkage after healing.At present,they are mostly used in burns and plastic surgery.The wounds formed are mostly scar healing,which may cause itching and pain in patients Symptoms have an impact on patients' lives,and currently there are fewer treatments for wounds in the donor site.The purpose of the author's study was to observe the therapeutic effect of PRF on wound donor area of fault skin through animal experiments and clinical applications,and to confirm that PRF has a good and beautiful promoting effect on wound donor area of fault skin.Methods:(1)Animal experiment: 10 healthy New Zealand white rabbits weighing(2.7-3.1)kg / head were used,and three-square tomographic skin patches of about 2.5 cm * 2.5 cm in size were made on the back of each rabbit.Blank group,recombinant human basic fibroblast growth factor gel Recombinant Human Basic Fibroblast Growth Factor(rh-b FGF)control group,PRF experimental group,the wound healing was observed,and HE staining was performed one month later to observe the operation area Histological morphology.(2)Clinical application: According to the exclusion criteria,40 patients with medium-thickness skin grafts who were hospitalized in plastic surgery at the Second Affiliated Hospital of Anhui Medical University from November 2018 to May 2019 were randomly divided into experimental groups(n = 20)and the control group(n = 20).The experimental group was treated with PRF to treat the wound skin donor site,and the control group was treated with rh-b FGF.Record wound infection,healing time,and fourpoint verbal rating scale(VRS-4)every 3 days for pain assessment,Vancouver Scar Scale(VSS)assessment,and record the 6th postoperative score.Month scar condition.Results:(1)Animal experiments: The average healing time of rabbit wounds was in experimental group < control group < blank group(P <0.05);HE staining showed that collagen fiber proliferation in experimental group was significantly less than that in control group and blank group.(2)Clinical application: The wound healing time of rhb FGF group was longer than that of PRF group(17.80 ± 0.95 d vs 16.75 ± 0.91d)(P <0.05);there was no statistical difference in wound infection rate between the two groups(P?0.05);The rh-b FGF group had a higher VRS-4 score than the PRF group.The differences between the two groups on the 3rd,6th,and 9th days were statistically significant(P <0.05),and there was no statistical difference on the12 th,15th,and 18 th days after the operation(P?0.05);the VSS score of rh-b FGF group(3.00 ± 0.86)was higher than that of PRF group(2.00 ± 0.65)at 6 months after operation(P <0.05).Conclusion: PRF can promote wound healing in the donor site of torn skin and improve postoperative pain and scar formation.
