Study On The Model Of Chinese Medical Association Classification Diagnosis And Treatment

With the social and economic development of our country,people's demand for health is increasing rapidly,and the demand is constantly enriched.How to coordinate regional medical resources and build a reasonable and stable pattern of medical treatment is a huge challenge for the existing medical and health service system in our country.In April 2009,China promulgated the opinions of the CPC Central Committee and the State Council on deepening the reform of the medical and health system,the state launched a new round of deepening the reform of the medical and health system(new medical reform),requiring medical and health units at all levels to actively promote the hierarchical diagnosis and treatment work with the primary diagnosis and two-way referral as the main content.In 2010,the original Ministry of health and other five ministries and commissions issued the guidance on the pilot reform of public hospitals,which requires the overall coordination of medical resources at all levels of medical institutions,and promote the implementation of hierarchical diagnosis and treatment.In 2012,the 12 th Five Year Plan for the development of health services proposed to identify the functional orientation of medical institutions at all levels and promote the technical and resource assistance of medical institutions above the second level to grass-roots medical institutions.In September 2015,the general office of the State Council issued the guiding opinions on promoting the construction of hierarchical diagnosis and treatment system,which clearly required to improve the ability of grass-roots medical services,implement hierarchical diagnosis and treatment,and establish a hierarchical diagnosis and treatment system in line with the national conditions.The establishment and improvement of the hierarchical diagnosis and treatment system is conducive to the overall planning of medical resources,improving the diagnosis and treatment capacity at the grass-roots level,promoting the rational use of high-quality medical resources,and standardizing the pattern of medical treatment.Through literature research,this paper summarizes the research on hierarchicaldiagnosis and treatment carried out by experts and scholars at home and abroad,and finds that the current research on hierarchical diagnosis and treatment mainly focuses on connotation research,international experience research,current situation research,problems and countermeasures research,as well as practical evaluation research.Under the guidance of interest group theory and resource integration theory,this paper summarizes five main measures to implement hierarchical diagnosis and treatment in w City through empirical research: first,to improve the service capacity of grass-roots medical institutions;second,to strengthen the rigid guidance of medical insurance policy;third,to strengthen the flexible guidance of hierarchical diagnosis and treatment publicity;fourth,to standardize the hierarchical diagnosis and treatment system;fifth,to strengthen the hierarchical diagnosis and treatment assessment.Based on the analysis of the current situation of graded diagnosis and treatment in w City,it is found that there are some problems in graded diagnosis and treatment in w City,such as the shortage of health technical personnel in basic medical institutions,the lack of medical equipment,the low utilization rate of beds in basic hospitals,the decrease of two-way referral rate,the low rate of first diagnosis at the basic level of patients,and the inaccurate functional positioning of municipal hospitals.Through the analysis of patients' and medical staff's cognition and satisfaction of graded diagnosis and treatment in w City,this paper adopts the method of questionnaire survey.It is found that: first,more than half of the patients don't know the specific content of graded diagnosis and treatment,and most of the medical staff can know the content of graded diagnosis and treatment;second,whether the waiting time,medical equipment configuration are complete and timely The main reasons that affect the patients' satisfaction with classified diagnosis and treatment are to judge the condition of illness and the continuity of diagnosis and treatment;the third is that the working environment is good or bad,the medical equipment is complete,the medical information is shared,and the income level is high or low;the fourth is that the main reasons that make it difficult for the grass-roots hospitals to absorb medical talents are the low income level The fifth is that 80% of medical staff think that we should give full play to the advantages of medical resources as a whole,and increase the investment in personnel training,medical equipment,diagnosis and treatment environment,technical guidance and other aspects.In addition,through literature research,this paper summarizes and draws lessons from foreign experience of hierarchical diagnosis and treatment represented by the United Kingdom,Japan and the United States,and analyzes and compares the five domestic models of hierarchical diagnosis and treatment.The article concludes that there are five problems in W city's Medical Association classification diagnosis and treatment,one is the poor publicity effect of classification diagnosis and treatment,the other is the low rate of first diagnosis at the grass-roots level,the third is the insufficient medical service capacity of grass-roots hospitals,the fourth is the inaccurate functional positioning of municipal hospitals,and the fifth is the imperfect two-way referral system.In view of the problems existing in the hierarchical diagnosis and treatment of medical consortia in w City,this paper puts forward that: we should further strengthen the publicity,enhance the awareness of the masses;increase the investment in medical equipment,strengthen the construction of talent team,improve the medical service ability of grass-roots hospitals;improve the hierarchical diagnosis and treatment system,especially the two-way referral system;break the inherent interest relationship,return to the functional orientation of the hospital,etc.,and improve the classification of W city Level diagnosis and treatment mode.And standardize the internal operation mechanism from six aspects,such as research on hierarchical diagnosis and treatment,medical insurance payment,talent introduction and training,clinical management,medical teaching and scientific research management.There are three innovations in this paper: first,a comparative analysis of the domestic and foreign representative hierarchical diagnosis and treatment mode;second,a questionnaire survey on the cognition and satisfaction of patients and medical staff on hierarchical diagnosis and treatment is carried out to analyze the effect of hierarchical diagnosis and treatment in w City;third,the excellent experience of domestic and foreign hierarchical diagnosis and treatment mode is summarized,the hierarchical diagnosis and treatment mode in W city is innovated and constructed,and the internal operation mechanism is standardized.This paper makes up for the lack of research and analysis of the hierarchical diagnosis and treatment model,which has certain practical significance and reference significance for the research of the hierarchical diagnosis and treatment model in China.
