Objective: By analyzing the level of thrombin-activated fibrinolytic inhibitor,the level of high-sensitivity C-reactive protein and the relationship between the two indexes and microalbuminuria excretion rate in patients with type 2 diabetes mellitus,the role of plasma TAFI and hsCRP in the pathogenesis of microalbuminuria in type 2 diabetes mellitus is evaluated,thus providing a new theoretical basis for clinical early intervention and treatment in type 2 diabetes mellitus complicated with microalbuminuria group.Methods:According to the clinical diagnostic criteria of type 2 diabetes in China,Sixty-five patients with type 2 diabetes diagnosed in the outpatient or in-patient department of Heping Sub-hospital Of The Northern Theater General Hospital from May 2018 to June 2019 were selected into the case group.According to the measurement results of urinary albumin excretion rate,the subjects selected into the case group were divided into the normoalbuminuria group of 30 patients,16 males and 14 females,aged(51.22±11.96)years;35 patients were selected to the microalbuminuria group,including 22 males and 13 females,with age of(55.15±13.37)years.Thirty patients who were completely healthy after examination were selected into the normal control group,including 17 males and 13 females,with an average age of(53.26 ± 10.31)years.The basic indicators of the case group and the control group were matched.All subjects excluded acute and chronic infections,inflammatory diseases such as rheumatism,tissue damage and necrosis,malignancy and surgery,and other diseases other than type 2 diabetes such as primary cardiovascular and cerebrovascular diseases and primary liver and kidney diseases in the recent(1 month)period.UAER was determined by radioimmunoassay,hsCRP was determined by immunoturbidimetry,and TAFI was detected by enzyme-linked immunosorbent assay.Spss21.0 statistical software was used to analyze the obtained data,The measurement data was expressed as mean ± standard deviation(x ±s),and the normal distribution of the data was analyzed.The data of skewed distribution is converted to normal distribution by logarithm,One-way ANOVA,LSD test,t test and t'test were used for the significance of the difference.Spearman correlation coefficient was used to test the relationship between plasma TAFI,hsCRPand UAER.Results: 1.Plasma TAFI,hsCRP levels were 138.72±32.54%,5.86±0.89mg/L and83.93±19.75%,1.13±0.36mg/L in the type 2 diabetes group and the normal control group,respectively.TAFI and hsCRP levels in the type 2 diabetes group were significantly higher than those in the control group,with significant differences(p<0.001).Plasma TAFI and hsCRP levels were 154.32±22.16%,8.23±0.96mg/L and 123.12±14.32%,3.49±0.83mg/L,respectively,in the microalbuminuria group with type 2 diabetes and the normal albuminuria group with type 2 diabetes,and TAFI and hsCRP levels were significantly higher in the microalbuminuria group with type 2 diabetes than in the normal albuminuria group with type 2 diabetes(p<0.005).2.plasma TAFI and hsCRP were significantly correlated with UAER in patients with microalbuminuria in type 2 diabetes(p<0.001).Conclusion: 1.Plasma TAFI and hsCRP levels were increased in patients with type 2diabetes mellitus complicated with microalbuminuria.2.The occurrence of microalbuminuria in type 2 diabetes mellitus is related to the increase of TAFI and CRP levels in plasma.3.In the patients with type 2 diabetes,the hypercoagulable state is caused by both coagulation activation(increased plasma FIB level)and fibrinolytic inhibition(increased plasma TAFI level),suggesting that the patients with type 2 diabetes should be clinically treated with appropriate anticoagulant therapy.4.Plasma TAFI and hsCRP levels could be used as a judgment index of disease progression and disease severity of type 2diabetes complicated with microalbuminuria.
