Objective:To observe the levels of soluble suppression of tumorigenicity 2(s ST2)in patients with left heart failure combining different degrees of pulmonary hypertension and to explore its clinical significance.Methods:A total of 120 patients with chronic left heart failure were selected,who were treated in our hospital from November 2017 to April 2019.According to the results of ultrasound cardiogram,pulmonary arterial systolic pressure(PASP)>35 mm Hg(1 mm Hg =0.133 kpa)was used as the basis for the diagnosis of pulmonary hypertension(PH).It was divided into three groups,40 patients with simple heart failure group(PASP?35mm Hg),40 patients with mild pulmonary hypertension group(36-49 mm Hg)and 40 patients with moderate and severe pulmonary hypertension group(?50 mm Hg).To collect the relevant clinical data and laboratory indicators,determine the plasma s ST2 levels by ELISA and perform the statistical analysis.Results:1 Comparison of baseline informationThere was no differences in age,sex,BMI,NYHA and serum uric acid in the three groups(P >0.05).Compared with simple heart failure group,the RDW and TB in both mild pulmonary hypertension group and moderate and severe pulmonary hypertension group had statistically significant difference(P<0.05).The difference of RDW and TB in mild pulmonary hypertension and in moderate and severe pulmonary hypertension group was statistically significant(P<0.05).2 Comparison of plasma levels of s ST2 and NT-pro BNPThe plasma levels of s ST2 in the group of simple heart failure,mild pulmonary hypertension and moderate and severe pulmonary hypertension were152.85(138.55,168.25)pg/m L,262.95(223.65,290.43)pg/m L,377.45(329.15,419.88)pg/m L.The plasma levels of NT-pro BNP were respectively(1893.23±1217.87)pg/m L,(2750.18±1541.92)pg/m L,(3720.38±1782.18)pg/m L.The difference between the three groups was statistically significant(P <0.05).3 Comparison of ultrasound cardiogram parametersLVEF was significantly decreased and LAD,LVD,RVD were increased in the moderate and severe pulmonary hypertension group comparing with the other two groups(P <0.05).LVEF of mild pulmonary hypertension group was lower than that of simple heart failure group,and LAD,LVD,RVD were higher(P <0.05).4 The correlation between plasma s ST2 and other indicesNT-pro BNP,LVD,LAD,RVD,RDW,TB,PASP had positive correlation with s ST2,and the correlation coefficients were respectively r1=0.689,r3=0.676,r4=0.649,r5=0.594,r6=0.334,r7=0.404,r8=0.712,(P<0.05),among which s ST2 has the highest correlation with PASP,and less correlation with RDW and TB.The plasma levels of s ST2 was negatively correlated with LVEF,r2=-0.674,(P <0.05).Conclution:1 The plasma levels of s ST2 in patients with left heart failure combining different degrees of pulmonary hypertension increased with the increase of pulmonary arterial systolic pressure.SST2 may be involved in the development of the disease.Detection of plasma s ST2 levels contributes to the diagnosis of left heart failure with pulmonary hypertension.2 The plasma levels of s ST2 were correlated with NT-pro BNP and ultrasound cardiogram parameters.Combining multiple indicators with ultrasound cardiogram can predict the poor prognosis of the disease.
