Objective: To research and discuss the protective effect of exogenous L-carnitine supplementation on renal function damage in rats with severe scald.Materials and methods According to the method of random number table,66 adult female SD rats were divided into three groups: normal control group,scalded group,and scalded + carnitine group,in which 6 rats in normal control group,30 rats in each of the scalded group and the scalded + carnitine group.Rats in the normal control group were immediately sampled and sacrificed after anesthesia.In the latter two groups,30% TBSA full-thickness scald was made on the back of rats,and 4 ml · kg-1·% TBSA-1(according to Parkland's rehydration formula)was injected into the tail vein immediately.At post injury hour(PIH)1,rats in scalded group and scalded + carnitine group were injected with normal saline and 400 mg · kg-1 L-carnitine solution respectively,and were injected every 24 hours.Except for the normal control group,6 rats were taken from each group to collect renal tissue and abdominal aorta blood at PIH 6,12,24,48 and 72.The serum contents of blood urea nitrogen(BUN),creatinine(CRE),total protein(TP),albumin(ALB),and cystatin C(Cys-C)were measured by automatic biochemical analyzer.The HE stained sections of rats kidney were observed under a microscope,and the morphological changes of each group were compared.One-way analysis of variance on data,multiple comparisons using Bonferroni correction.Results(1)The serum BUN content of rats in the scalded + carnitine group was significantly lower than that in the scalded group at PIH 6,48,72(P <0.05),and was similar to that of normal control group[6.36 ± 0.53)mmol / L,P > 0.05] at 72 h [(7.33 ± 0.48)mmol /L].The serum BUN content in scalded rats was significantly higher than that in the normal control group(P <0.05)at each time point after injury.The serum BUN content in the scalded group and the scalded + carnitine group showed a trend of increasing first and then decreasing,reached the peak value at PIH 12[(22.71 ± 3.95),(17.92 ± 2.00)mmol / L,respectively].(2)The serum CRE content in the scalded + carnitine group was significantly lower than that in the scalded group at PIH 12,24,48,and 72(P <0.05),and it was similar to the normal control group at PIH 24,48 and 72(P> 0.05).The serum CRE content of scalded rats was significantly higher than that of the normal control group(P <0.05)at each time point after injury.The serum CRE content in the scalded group and the scalded + carnitine group showed a trend of increasing first and then decreasing,reached the peak value at PIH 12 [(42.8±6.3),(33.3±3.4)?mol/L,respectively].(3)The serum TP content of scalded + carnitine group was significantly higher than that of scalded group(P < 0.05)at PIH 12 and 24,and was similar to that of normal control group [(68.3±4.8)g/L,P > 0.05] at PIH 6 [(63.7 ± 5.1)g/L].The serum TP content in the scalded group was significantly lower than that in the normal control group(P <0.05)at each time point after injury.The TP content in the scalded group and the scalded + carnitine group showed a trend of decreasing first and then increasing,reaching the lowest value at PIH 24 [(46.1±2.5),(54.7±5.0)g/L,respectively].(4)The serum ALB content in scalded + carnitine group [(35.2 ± 3.1)g / L] was significantly higher than that in scald group at PIH 12 [(29.8±2.3)g/L,P<0.05],and was similar to that in normal control group[(41.3±3.0)g/L,P>0.05] at PIH 6 [(38.1 ± 3.3)g/L].The serum ALB content of scalded rats was significantly lower than that of the normal control group(P <0.05)at each time point after injury.The serum ALB content of the scalded group and the scalded + carnitine group showed a trend of decreasing first and then increasing,reached the lowest value at PIH 24 [(29.9±1.9),(25.8±3.3)g/L,respectively].(5)The serum Cys-C content in scalded + carnitine group [(0.21 ± 0.04)mg/L] was significantly lower than that in scalded group at PIH 24 [(0.30±0.06)mg/L,P<0.05],and similar to that in normal control group at PIH 24,48 and 72(P > 0.05).The serum Cys-C content in the scalded group was significantly higher than that in the normal control group at PIH 6,12,24,and 48(P <0.05).The serum Cys-C content in the scalded group and the scalded + carnitine group showed a trend of increasing first and then decreasing,reached the peak value at PIH 12 [(0.33±0.07),(0.24±0.04)mg/L,respectively].(6)The degree of renal tissue injury in the scalded + carnitine group was lighter than that in the scalded group.The renal tissue of rats in the normal control group was basically normal at each time point after injury.Among them,at PIH 24,large area swelling,vacuolar degeneration and necrosis of renal tubular epithelial cells were seen in the scalded group,more than two-thirds of the renal tubular nuclei disappeared.In addition,the renal tubular lumen is narrow,necrotic exfoliated cells and protein casts can be seen in the lumen,and edema and inflammatory cell infiltration can be seen in the renal interstitial.However,in the scalded + carnitine group,edema and necrosis of renal tubular epithelial cells were significantly reduced,included the cast,and the inflammatory cells were less infiltrated.Conclusions Early supplementation of exogenous L-carnitine in severely scalded rats can not only reduce renal cell damage,but also effectively improve the filtration of TP and ALB in glomeruli and the reabsorption of renal tubules,and make BUN,CRE and Cys-C return to the normal level faster,which can effectively shorten the recovery period of acute renal function damage,and thus maintain the stability of early renal function metabolism in severely scalded rats.
