Objective: To observe the effect of early supplementation of exogenous carnitine on liver mitochondrial damage and autophagy in severely scalded rats and to explore its pathological mechanismMaterials and methods Seventy-two adult female Sprague-Dawley rats were divided int: sham injury(A),scald injury and scald injury+carnitine groups according to the random number table,with 24 rats in each group.Rats in group A were sham injured,while rats in group B and C were inflicted with 30% TBSA full-thickness scald on the back.Rats in group B and C were injected with lactated Ringer's solution with the dosage of 30 m L/kg via caudal vein immediately for liquid resuscitation according to the Parkland formula.At post injury hour(PIH)1,exogenous carnitine was supplemented with the dosage of 300mg·kg-1/d via the tail vein in scald injury+carnitine groups.At PIH Six hours 12,24,48 and 72,abdominal aorta blood and liver tissue were collected from 6 rats in each group.The serum levels of carnitine,?-hydroxybutyric acid and OCT(ornithine carbamoyltransferase)were determined with ELISA,and the serum levels of LDH,ALT,AST was determined by automatic biochemical analyzer,Pathological changes of rats liver tissue were detected with HE staining.Data were processed with ANOVA and SNK test or Tamhane test,Bonfferni correction was used.Results(1)Compared with group A,the serum levels of carnitine of rats in group B were significantly lower at each time point(P values below 0.05),and those of group C were significantly lower at the point of PIH 12,24 and 48(P values below 0.05),in addition,the serum levels of carnitine of rats in group C were similar to those of group A at the point of PIH 72(P values above 0.05).The serum levels of carnitine in group C were significantly higher than those of group B at each time point(P values below 0.05).The serum levels of carnitine in group B and group C both showed a trend of increase,and they peaked at PIH 72,respectively(16.79±1.29),(28.22±3.02)?g/ml.(2)The serum levels of ?-hydroxybutyric of rats in group B and C were significantly lower than those of group A at each time point(P values below 0.05).The serum levels of ?-hydroxybutyric in group C were significantly higher than those of group B at each time point(P values below 0.05).The serum levels of ?-hydroxybutyric in group B and group C both showed a trend of increase,and they peaked at PIH 72,respectively(1.77±0.30)?(2.93±0.44)mmol/L.(3)The serum levels of OCT of rats in group B and C were significantly higher than those of group A at each time point(P values below 0.05).The serum levels of OCT in group C were significantly lower than those of group B at each time point(P values below 0.05).The serum levels of OCT in group B and group C both showed a trend of decrease,and they peaked at PIH 12,respectively(186.28±6.77)?(163.38±9.34)ng/ml.(4)The serum levels of LDH of rats in group B were significantly higher than those of group A at each time point(P values below 0.05),and those of group C were significantly higher at the point of PIH 12 and 24(P values below 0.05),in addition,the serum levels of LDH of rats in group C were similar to those of group A at the point of PIH 48 and 72(P values above 0.05).The serum levels of LDH in group C were significantly lower than those of group B at each time point(P values below 0.05).The serum levels of LDH in group B and C both showed a trend of decrease,and they peaked at PIH 12,respectively(3075.0±510.22)?(2226.3±274.05)U/L.(5)The serum levels of ALT and AST of rats in group B and C were significantly higher than those of group A at each time point(P values below 0.05).The serum levels of ALT of group C were significantly lower than group B at the point of PIH 48 and 72(P values below 0.05),and the serum levels of AST of group C were significantly lower than group B at the point of PIH 48(P values below 0.05).The serum levels of ALT and AST in group B and C both showed a trend of decrease,and they peaked at PIH 12,respectively(288.17±36.71)?(1775.7±172.70)U/L,and(260.50±24.97)?(1510.7±145.14)U/L.Conclusions Due to the early supplementation of exogenous carnitine in severely scalded rats,Serum carnitine and ?-hydroxybutyrate concentrations can be restored to normal levels faster,reducing autophagy in liver cells,alleviate hepatic mitochondrial damage,and maintaining the metabolic stability of hepatocytes in early stage of severe scald.
