Objective To investigate the prognostic value of serum soluble suppression of tumorigenicity 2(sST2),Galectin-3(Gal-3)and growth differentiation factor-15(GDF-15)in patients with chronic heart failure(CHF),and to analyze the risk factors of prognosis in patients with CHF.Method80 patients with heart failure who were admitted to Anhui Provincial Hospital from May 2018 to August 2018 were selected.General clinical data of patients were collected,and peripheral serum sST2,Gal-3,and GDF-15 levels were measured by enzyme-linked immunosorbent assay(ELISA).All patients were followed up for 1 year after discharge,and were divided into event group(39cases)and non-event group(41 cases)based on the outcome of end-point events(cardiovascular death or readmission due to heart failure)after 1 year of follow-up.The serum sST2,Gal-3,GDF-15 levels and general clinical data were compared between the two groups at admission,and the prognostic value of serum sST2,Gal-3,and GDF-15 alone and combined detection in CHF patients was evaluated.Result1.There was no significant difference in gender,body mass index,blood sodium,low-density lipoprotein cholesterol ester(LDL-C),and left ventricularend-diastolic diameter(LVEDD)between the event group and the non-event group(all P> 0.05).Compared with the non-event group,patients in the event group were older,had a higher proportion of patients with New York Heart Association(NYHA)heart function III-IV classification,had a shorter6-minute walk,and had a left ventricular ejection fraction(LVEF)was lower,serum creatinine was higher,and hospital stay was longer(all P <0.05).2.Compared with the non-event group,the levels of sST2,Gal-3,GDF-15,and N-terminal B-type natriuretic peptide precursor(NT-pro BNP)in the event group were increased(all P <0.05).3.Correlation analysis showed that NT-pro BNP,sST2,Gal-3,and GDF-15 had a significant positive correlation with each other(all P> 0.01).The above four variables were positively correlated with NYHA cardiac function classification(all P> 0.01).),Negatively correlated with LVEF(all P <0.05),NT-pro BNP and sST2 were positively correlated with LVEDD(all P <0.01),and Gal-3,GDF-15 and LVEDD were not significantly correlated(all P> 0.05).4.The receiver operating characteristic curve(ROC curve)showed that the areas under the curve of serum NT-pro BNP,sST2,Gal-3,and GDF-15 on the prediction of cardiovascular death or readmission events in CHF patients were0.946,0.924,0.894,and 0.768,respectively.the area under the curve are all greater than 0.7,and the four markers have good predictive value.5.The area under the curve of the four combined detection is 0.972,and the area under the curve of the new heart failure markers sST2,Gal-3,and GDF-15 combined detection is 0.964.Comparing the difference between the area under the ROC curve of the combined test and the single test,the four combined tests were better than the single test,and the combined test of sST2,Gal-3,and GDF-15 was also better than the single test(P <0.05).6.COX regression analysis showed that NT-pro BNP and sST2 wereindependent risk factors for endpoint events in CHF patients within 1 year.Conclusion1.Serum sST2,Gal-3 and GDF-15 levels reflect the severity of heart failure.2.Serum sST2,Gal-3,and GDF15 as a new type of biomarkers have certain value for the prognosis of CHF.The combination of the three and the traditional marker NT-pro BNP can improve the prognostic value.3.NT-pro BNP and sST2 are independent risk factors for endpoint events in CHF patients within one year.
