Objective: Chronic rhinosinusitis(CRS)is a common disease in otolaryngology.Its immune and histopathological mechanism is complex.When the medical treatement is ineffective,it often needs surgery.Functional endoscopic sinus surgery(FESS)has been accepted by the international nasal science.With the deepening of CRS research in China,the operation strategy is more personalized and accurate.Balloon dilatation of the paranasal sinuses(BS)is a minimally invasive technique rising in recent years at home and abroad.On the basis of following the principle of ventilation and drainage of FESS operation to reconstruct the nasal cavity and sinuses,it has outstanding advantages in protecting the physiological mucosa,especially when the lesion sinus ostium is small,and the opening of traditional surgical tools will cause greater mucosal damage.In recent years,it is often used as an auxiliary technique in sinus surgery.Frontal sinus,as the difficulty of sinus surgery,is often regarded as the most ideal object of BS.This essay discusses the efficacy and safety of balloon dilatation of the paranasal sinuses in the treatment of frontal sinusitis,and evaluates its clinical application value.Methods: The data of 21 patients(42 sides)with chronic sinusitis who underwent endoscopic assisted balloon dilatation of frontal sinus were analyzed retrospectively.Patients were divided into chronic sinusitis without nasal polyps(CRSsNP)and chronic sinusitis with nasal group Polyps(CRSwNP)were used to observe and record the location of the frontal sinus,the opening of the ostium of the frontal sinus,the Lund Kennedy endoscopic mucosal morphology score and the occurrence of complications.Results: 1 The total improvement rate of frontal sinus diameter was 42.86% at 2 weeks,35.71% at 6 weeks and 57.14% at 12 weeks.There was no significant difference between CRSsNP group and CRSwNP group(P > 0.05).2 Lund Kennedy endoscopic mucosal morphology score: the scores of 2 weeks,6 weeks and 12 weeks after operation were 3.36 ± 2.93,2.93 ± 1.16 and 1.79 ± 1.41(unit: mm),which were lower than 3.79 ± 1.26(P < 0.05).The scores of observation points in CRSsNP group were 2.94 ± 1.00,2.37 ± 0.50 and 1.44 ± 1.46 respectively,and those in CRSwNP group were 3.62 ± 0.64,3.27 ± 1.31 and 2.00 ± 1.36 respectively.There was no significant difference between the two groups(P > 0.05).Endoscopic observation showed that the morphology of the frontal sinus mucosa in CRSwNP group was better than that in other parts of the operation cavity at 6 weeks after operation.3 There were no complications such as frontal sinus closure,orbital swelling,massive hemorrhage and cerebrospinal fluid rhinorrhea in all patients after the operation.During the follow-up period,there was no need to correct the operation again.Conclusion: In conclusion,balloon dilatation of the sinuses is safe and effective for the treatment of frontal sinusitis.It is accurate and minimally invasive to locate the frontal sinuses during the operation.It can open the frontal sinuses with small diameter ostium and ensure the integrity of the mucosa as much as possible.As a supplementary technology of endoscopic frontal sinus surgery,it has advantages and is worth clinical promotion.
