Objective:To observe the effect of the modified extended endoscopic sinus surgery on frontal sinus drainage and olfaction in patients with nasal polyps and allergy,and to evaluate its clinical effect,so as to lay a foundation for expanding its clinical application.Methods:43 patients with chronic rhinosinusitis with bilateral nasal polyps and allergy treated in our hospital from January 2017 to December2018 were divided into the protected Middle turbinate-Extensive Endoscopic Sinus Surgery(mEESS)group(observation group)(n=23)and functional endoscopic sinus surgery(FESS)group(control group)(n = 20).The severity of the disease before surgery and the clinical results of 6months and 1 year follow-up were compared and evaluated between the two groups by visual analogue scale(VAS),Lund-Mackay(L-M)score of nasal sinus CT and nasal endoscopic Lund-Kennedy(L-K)score.Results:The general condition,VAS score,L-M score and L-K score of the two groups were similar(p>0.05).The overall symptoms and VASscores of each symptom and L-K score of the two groups were improved at6 months and 1 year after operation compared with that before operation(P<0.05).The olfactory function of the observation group was significantly better than that of the control group at 6 months after operation(P=0.047< 0.05).The patency rate of frontal sinus orifice in the observation group was significantly higher than that in the control group at6 months and 1 year after operation(P < 0.05).There was no significant difference in postoperative complications between the two groups,and the satisfaction of the observation group was higher than that of the control group(P=0.048<0.05).Conclusion: Compared with FESS,the modified "protected Middle turbinate-Extensive Endoscopic Sinus Surgery" can better ensure frontal sinus drainage,improve postoperative satisfaction and improve olfactory sense in the short term,but the long-term effect needs further observation and study.
