| Objective:To observe the distribution of HPV in cervical condyloma acuminatum?CA?,so as to clear the necessity of cervical HPV detection and follow-up among cervical CA patients.To investigate the clinical cure rate,the verrucous body clearance rate,the recurrence rate,HPV negative conversion rate and the adverse reaction rate of CO2 laser combined with ALA-PDT in the treatment of cervical CA,so as to comprehensively evaluate the method of clinical efficiency.Methods:Patients who were treated with cervical CA from August 2017 to September 2019.The method of polymerase chain reaction?PCR?was used to detect cervical HPV genotypes of cervical CA patients.The test results were analyzed.All are randomly divided into experimental group and control group.The experimental group was treated with CO2laser and ALA-PDT,while the control group was treated with ALA-PDT.7-10d repeated treatment 1 time,a total of 3 times,then follow up 3 months.The clinical cure rate and wart clearance rate of the two groups were evaluated at 1 week after the last treatment;and the recurrence rate,HPV negative conversion rate and incidence of adverse reactions were evaluated at 3 months after the last treatment.Results:1.Distribution of HPV infection in Cervical CAThere were 46 cervical CA patients in total,all of whom were positive with the positive HPV infection rate of 100.00%.There were 29 cases of single HPV infection with infection rate of 63.04%,and 17 cases of multiple HPV infection with infection rate of 36.96%.The occurrence of low risk HPV and high risk HPV were 46 times and 23times,respectively.The proportion of low and high risk HPV was 2:1.Among 46HPV-positive patients,the top five HPV genotypes were HPV11?HPV6?HPV18?HPV16?HPV52.2.Comparison of the clinical cure rates after 1 week of the last treatmentIn 23 cases of experimental group,all are recovered?100.00%?;In 23 cases of control group,there are 16 cases recovered?69.57%?.The clinical cure rate of the experimental group was higher than that of the control group,and the two groups of the cure rate difference between was statistically significant??49??27?0.05?.3.Comparison of the verrucous body clearance rate after 1 week of the last treatmentThe number of warts in the experimental group was 50,and that in the control group was 42.The verrucous body clearance rate of two groups were 100.00%and 80.95%, respectively.The verrucous body clearance rate of the experimental group was higher than the control group,and the difference between the two groups was statistically significant??49??27?0.05?.4.Comparison of the recurrence rate after 3 months of the last treatmentIn 23 cases of experimental group,there is 1 case recurred?4.35%?;in 16 cases of control group,there are 5 cases recurred?31.25%?.The recurrence rate of the experimental group was lower than that of the control group,and the difference was statistically significant between the two groups??49??27?0.05?.5.Comparison of the negative conversion rates after 3 months of the last treatmentThe negative conversion rate of HPV infection in the experimental group and the control group were 91.30%?21/32?and 56.25%?9/16?,respectively.There was statistically significant difference between the two groups??49??27?0.05?.6.Comparison of the adverse reaction rateThe rate of adverse reactions in the experimental group was higher than that in the control group,but no statistically significant differences between the two groups??49??27?0.05?.Conclusions:1.The main infection of cervical CA patients is single HPV infection,and at the same time multiple HPV infection is common.Cervical CA was mainly low risk HPV infection,supplemented by high risk HPV infection.The most popular low risk HPV genotypes were HPV11 and HPV6,and HPV18?HPV16?HPV52 of high risk HPV.2.CO2 laser combined with ALA-PDT in treating cervical CA is superior to ALA-PDT in clinical cure rate,wart clearance rate,recurrence rate and negative conversion rate,and has no serious adverse reaction,which has accurate clinical effect. |
