Objective:The present study aimed to evaluate the application of fine-needle aspiration cytology(FNAC)of the thyroid coupled with the screening of BRAFV600 E mutation in the detection of papillary thyroid carcinoma(PTC),in order to increase the preoperative diagnostic accuracy rate of papillary thyroid carcinoma.Methods:From October 2017 to June 2019,the FNAC specimens of the 996 cases pathological results were papillary thyroid carcinoma and 248 cases NG were collected from the Pathology Department of the First Central Hospital of Baoding.BRAFV600 E mutation was screened in all the liquid-based specimens by using quantitative fluorescence PCR.Combined with the pathological results after surgery,the value of FNAC combined with BRAFV600 E screening results in the preoperative diagnosis of papillary thyroid carcinoma were compared and analyzed.The statistical analyses were performed using Statistical Package for the Social Sciences(SPSS),version 20(SPSS Inc.,Chicago,IL,USA).Chi-squared(?2)test was used to calculate the sensitivity,specificity,and accuracy of two PTC detection methods that either uses FNAC results alone or employing the results of both FNAC and BRAFV600 E mutation screening.P < 0.05 is considered statistically significant.Results:1 The results of the FNAC diagnosisOf the 996 cases of the pathological results were papillary thyroid carcinoma,610 cases of PTC and 10 cases of benign were diagnosed definitely by FNAC;and 376 cases were diagnosed indefinitely including 90 cases atypical cells,63 cases follicular tumors/suspected follicular tumors and 223 cases of suspected PTC;In the 248 cases of the pathological results were NG,198 cases benign were diagnosed definitely by FNAC,and 50 cases were diagnosed indefinitely including 28 cases atypical cells and 22 cases follicular tumors/suspected follicular tumors.2 Detection of the BRAFV600 E mutationCombined with BRAFV600 E screening,580 cases with definite diagnosis of PTC and 186 cases with indefinite diagnosis of the PTC by FNAC were express positive,All the benign cases whether definite or indefinite diagnosis by FNAC were express negative.3 Sensitivity,specificity,and accuracy of using fine-needle aspiration cytology(FNAC)alone and combined application of FNAC with BRAFV600 E mutation screening for detecting papillary thyroid carcinoma.The sensitivity,accuracy and specificity of the FNAC alone in the diagnosis of PTC were 62.24%,79.83%,and 64.95%,respectively.On the other hand,the sensitivity,accuracy and specificity of FNAC combined with BRAFV600 E mutation screening in the diagnosis of PTC were 76.90%,100%,and 77.49%,respectively.The sensitivity,specificity and accuracy of FNAC combined with BRAFV600 E mutation screening in the detection or diagnosis of PTC were significantly higher(P < 0.001).4 The relationship between the mutation of BRAFV600 E and the pathological parameters.There was no statistically significant difference between the mutation of BRAFV600 E and age,sex,tumor size and lymph node metastasis(p >0.05).Conclusion:1 Combined with BRAFV600 E detection,the preoperative detection rate of PTC can be increased.2 There was no significant difference between age,sex,tumor size and lymph node metastasis and BRAFV600 E mutation(P>0.05).
