Purpose: through the fine needle aspiration biopsy(FNAB)and serum thyroid stimulating hormone(TSH)texting of the suspicious thyroid carcinoma,to explore the diagnostic value in papillary thyroid carcinoma.Methods: 235 cases of thyroid nodules hospitalized in the first People's Hospital of Xiangtan were recruited from February 01,2016 to January 31,2018,according to the inclusion criteria,exclusion criteria and elimination criteria established by the study,150 cases were actually eligible for the selected specimens.serum TSH,Anti-thyroglobulin antibody(TGAb),anti-thyroid peroxidase antibody(TPOAb)were measured in all patients before operation,Preoperative color Doppler ultrasonography and fine needle aspiration biopsy were performed.By comparing the FNAB results and the postoperative routine pathological result,divided into positive group and negative group,the false positive and false negative group.Using SPSS 19.0 statistic software,the TSH level,TGAb,TPOAb,age,gender in different result group were statistically analyzed(P<0.05 expressed statistically significant difference),and the risk factors analysis of thyroid malignant lesions use the Logistic regression analysis,draw ROC curve to examine the diagnostic value of benign and malignant thyroid nodules on FNAB combined with preoperative serum TSH.Results: The difference of total serum TSH between benign thyroid nodule patients and malignant patients after operation was statistically significant(P<0.05).The preoperative FNAB positive group was significantly higher than the negative group,the difference was statistically significant(P<0.05).There was no significant difference in preoperative TSH levels between the positive group and the false negative group(P>0.05).There was a statistically significant difference in preoperative TSH levels between the negative group and the false negative group(P<0.05).Serum TGAb levels were significantly different between benign and malignant groups(P<0.05),while TPOAb levels were not significantly different(P>0.05).Logistic regression analysis showed that serum TSH level was an independent risk factor for malignant lesions of thyroid nodules(P<0.01,OR=1.941).The ROC curve(receiver operating characteristic curve)analysis showed that the thyroid FNAB examination combined with preoperative TSH levels has a significantly improvement of the diagnostic accuracy of thyroid nodule.Conclusions: 1.Serum TSH levels are associated with the occurrence of malignant lesions of thyroid nodules,The higher the level of TSH,The greater the risk of thyroid malignancy.2.The increase of TPOAb level may be related to the occurrence of thyroid malignancy.3.Combined with TSH level can improve the accuracy of FNAB diagnosis of papillary thyroid carcinoma to some extent.4.The level of TSH can be used as an independent predictive risk factor and an auxiliary diagnostic indicator for malignant thyroid nodules.
