In recent years,with the improvement of people's health consciousness,the requirement of operation is also to rise,for lumbar spinal stenosis patients with central stenosis,the operation are not limited to the purpose of thorough decompression,but also should try to meet the demand for small damage,less blood loss and other requirements,as a result,the traditional Total laminectomy for decompression is facing the challenge of many emerging ways of minimally invasive surgery,the bridge decompression not only can thoroughly remove oppression,but have the advantage of less injury,less damage to spinal rear structure,etc.Some studies have shown that patients undergoing bridge decompression perform better than the counterparts of traditional total laminectomy decompression,and many experts still support total laminectomy decompression as the best surgical treatment for lumbar spinal stenosis.Therefore,we aimed at the postoperative efficacy and other indicators of the two surgical methods to study which surgical method is more suitable for patients with central stenosisObjective:To explore the advantages and disadvantages of bridge decompression and total laminectomy decompression in the treatment of lumbar spinal stenosis with central stenosisMethods:from January 2014 to June 2019,84 patients(39 males and 45 females)underwent decompression for lumbar spinal stenosis.Bridge Decompression was performed in 42 cases and total laminectomy was performed in 42 cases.Visual analogue scale(Vas),ODI scale(Odi)and Nakal scale(Nakal)were used to evaluate the curative effect of the two methods At the same time,blood loss,operation time and postoperative drainage volume were compared to evaluate the degree of injury.Results:All the 84 patients were followed up for more than 10 months.The Vas and Odi scores in bridge decompression group and total laminectomy decompression group were significantly improved compared with those before operation.Postoperative VAS and ODI were similar between the two groups.Bridge decompression was significantly less than total lamina decompression in terms of intraoperative blood loss,duration of operation and postoperative drainage volume,and the difference was statistically significant.In the Nakal classification,the excellent and good rate of bridge decompression group was 95.2%,and that of whole lamina group was 78.6%.The difference was significant.(p<0.05)Conclusion:In the surgical treatment of lumbar spinal stenosis with central Canal Stenosis,compared with total laminectomy decompression,bridge decompression has the advantages of less blood loss,shorter operation time,less injury to patients,better results and fewer complications.
