| Objective:To explore the clinical effect of PELD on the treatment of prolapsed lumbar disc herniation. Methods:In our retrospective analysis between June 2015 and June 2017,42 patients with prolapsed LDH received PELD,24 men and 18 women, with a mean age of 42.81±13.63(range16-69)years.All patients underwent preoperative lumbar dynamic radiographs to exclude cases of segmental instability, and had obtained preoperative and postoperative MR images. This study uses TESSYS technique for prolapsed lumbar disc herniation.Clinical outcomes were assessed with the visual analogue scale(VAS)score, the modified Mac Nab criteria, and the Oswestry disability index(ODI).Relevant data such as preoperative and postoperative patients with immediate,3 months,6 months,1 year rate, evaluating the clinical effect of PELD on the treatment of prolapsed lumbar disc herniation. Results:All patients were followed up range from a minimum of 6 to 24 months(mean 15 months).95.2%good to excellent result was obtained as per Macnab criteria,of which 35 cases of excellent, good in 5 cases, general in 2 cases, poor in 0 cases.Preoperative VAS score was 7.79 ±1.07,immediate postoperative was 3.88± 0.77,1 month was 2.36± 1.38,3 months was 1.17± 1.10,6 months was 0.50± 0.77,1 year was 0.24± 0.43,were significantly lower than preoperation. The ODI improved from preoperative 0.70 ±0.08 to 0.05 ± 0.05 at last follow-up.The JOA improved from postoperative 7.48±1.55 to 26.29±1.33 at last follow-up(P < 0.05).Conclusion:TESSYS technique is a safe and effective method for the treatment of prolapsed LDH,which has the advantages of mild tissue injury, short hospital stay and fewer complications. |
