Effects Of ?-3 PUFAs On Peripheral Blood T Lymphocyte Subsets And Immunoglobulin In Patients With Gastric Cancer After Surgery

Objective: To study the changes of peripheral blood T lymphocytes(CD3~+,CD4~+,CD8~+,CD3~+/CD8~+)and immunoglobulin(Ig A,Ig G,Ig M)in patients with gastric cancer before and after surgery,as well as effects of postoperative application of omega-3 polyunsaturated fatty acids on their levels and its clinical application value.Methods: 90 patients with gastric cancer were collected who undergone radical resection in the Third Department of Surgery,the Fourth Hospital of Hebei Medical University from March 2019 to January 2020.According to the evaluation criteria of NRS2002 and PG-SGA,the patients were divided into malnutrition group(group A,n=50)and non-malnutrition group(group B,n=40).Patients in the malnutrition group(group A)were given preoperative enteral nutrition support.The patients in group A and group B were randomly divided into the experimental group(group A1,n=25 cases;group B1,n=20 cases)and the control group(group A0,n=25 cases;group B0,n=20 cases).Patients in group A1 and group B1 were given parenteral nutrition therapy containing omega-3 polyunsaturated fatty acids,however,patients in group A0 and group B0 were given parenteral nutrition therapy without omega-3 polyunsaturated fatty acids.Peripheral blood T lymphocytes,immunoglobulins,and other laboratory tests were performed before surgery,on the first postoperative day,as well as on the seventh postoperative day.Results:1.Patients with malnutrition(group A)had lower peripheral blood CD3~+,CD4~+,CD8~+,Ig A,Ig G and Ig M before surgery than those without malnutrition(group B)(P<0.05),and CD4~+/CD8~+ also showed a decreasing trend(P> 0.05).2.In the A and B groups of patients,CD3~+,CD4~+,CD8~+,Ig A,Ig G and Ig M were significantly reduced on the first day after surgery(P <0.05).CD4~+/CD8~+ in group B was significantly lower than that before surgery(P<0.05),and CD4~+/CD8~+ in group A was lower than that before surgery(P>0.05).3.The CD3~+,CD4~+,CD8~+,CD4~+/CD8~+,Ig A,Ig G and Ig M of patients in group A1 were significantly higher than those in group A0 on the seventh day after surgery(P <0.05).The CD3~+,CD4~+,CD8~+,CD4~+/CD8~+,Ig A,Ig G and Ig M of patients in group B1 were significantly higher than those in group B0 on the seventh day after surgery(P<0.05).CD3~+,CD4~+,CD8~+,CD4~+/CD8~+,Ig A,Ig G and Ig M in the four groups of A1,A0,B1,and B0 on the 7th day after surgery were all higher than those on the first day after surgery(P>0.05).4.Patients with postoperative complications of gastric cancer had significantly lower preoperative CD3~+,CD4~+,Ig A,Ig G and Ig M than patients without postoperative complications(P<0.05).5.The postoperative hospital stay of patients with ?-3 PUFAs was significantly shorter than that of patients without ?-3 PUFAs(P<0.05)in patients without postoperative complications.Conclusions:1.Patients with gastric cancer accompanied by malnutrition have reduced T-cell and humoral immune functions.2.Surgical trauma suppresses T cell immunity and humoral immunity in patients with gastric cancer.3.With the postoperative recovery of gastric cancer patients,T cell immunity and humoral immune function have been improved.The immune function has been improved significantly after the application of ?-3 PUFAs.4.Patients with gastric cancer accompanied by malnutrition have more complications due to decreased immune function.5.Application of omega-3 PUFAs immunonutrition treatment can reduce the hospital stay for patients and accelerate recovery.
